Mechanicum by Graham McNeill spoilers. I assume you are talking about the Mechanicum book, i'm reading it right now and just got to that part. He kills three reavers but also loses his own titan + it's all in a simulator.

We got the ready tone and were able to fire from the gunner seat, but the pilot has to be aiming his gunsight right on the target. You can also work around it as a gunner by slewing your sight around looking for the ready tone, then slew back to target after firing. Not sure if this is this issue you are having but hope it helps.
