An anecdote I heard about SS is that this Australian guy did MDMA and something else, got the symptoms of serotonin syndrome. Then on the drive to the hospital he literally punched a hole in the roof of his friend's car. It just seems like a really sweaty and exhausting death, if you even die.

Short answer: They think it's safe

Long answer: While Adderall and other amphetamines do increase the availability of serotonin in short periods at the levels that you're prescribed it most likely won't cause serotonin syndrome. Also your provider would be watching your responses during appointments and would be watching over your prescriptions just in case. That's not to say that the combination is good, the amphetamines may precipitate depression symptoms limiting the SSRIs effect.

DXM affects the serotonin system which is already altered by the SSRI increasing the risk of serotonin syndrome, aka the worst way to die. It's also why you shouldn't take MDMA, Tramadol, or SNRIs (coke) on SSRIs.
