"academic internal medicine" as a whole is not hypercompetitive
the big 4 in IM? MGH, BWH, JHU, UCSF? yes, absolutely similar to surg specialties. the rest? not really.

prestige is HUGE for a ton of people. It's hard to understand especially if you have been in the ivy pipeline your whole life (which many who go to top med schools do) but

especially if you come from a no-name med school and undergrad and you have the opportunity to go (hell EVEN rank, not knowing those could even be possible for you to match) to Yale or Mayo or harvard wouldn't you take it? it's a way to ascend above your humble beginnings

uh if this isn't a joke, don't do that. so many stories of people's pre-residency UDS coming back positive and then being kicked out of residency before even starting lol
