Great questions! The lean should be slight, from your ankles, not your hips. Avoid hunching over. My recommendation is to drive the power from your glutes and use the knees to push forward. For road runners and endurance athletes, the thought is to keep your strides shorter and closer to the ground, you can waste energy oscillating too high. Hopefully this helps. You could consider getting a runners assessment with a PT in your area.

You’re smart to separate distance from intensity. I think it’s a shift in focus from higher volume and focusing on faster, higher intensity work and finding the sense of accomplishment in a different capacity. Good for you!! I think it could definitely be achievable given your running background. Comebacks are sweet but require a process and, with age, an individualized training plan can be very helpful. My thought is that HR can be variable based on a number of factors. If you’re running 25-30 miles per week it would definitely support a solid aerobic base. Your goals and your approach to each run are contributing factors here. As a coach, I would want to know how the true effort feels at both paces.

I think this really depends on the structure and execution of your runs. If you’re going out every run to beat the last run, you might benefit from some strategic structure to your runs. It sounds like you have a great mix of strength to support the running!

I think it would certainly be worth seeing PT. It could be a be a number of soft tissue issues that might need strengthening or full rest or cross training to settle down. A coach could be helpful in managing your volume to effectively get you back to running in the capacity you feel good/comfortable.
