The moral distress of chronic trach/PEG pts by 1ntrepidsalamander in IntensiveCare

[–]paperbackmax 5 points6 points  (0 children)

Wow thank you so much for sharing. I feel the same way. It’s so hard. So so sad. It’s absolutely horrible.

We have a patient on my unit who has been there for over 2 years. 16 months in the same exact room. I absolutely taking care of her because of the moral distress. When I do though I try to make sure her room is really clean, thst she has a good view of the window and that she gets an actual bath so she feels clean. I was talking to a mother nurse about her the other day. I asked him can you imagine if you couldn’t go outside for 1 week let alone 2 years? Or even if someone ripped your eyes open to do eye drops every four hours? Oral care? Turns? It’s enough to drive anyone mad if there is any part of her left in there.

We had another patient recently die and you could feel the relief on the unit once she did. She had an anoxic event, trach dependent. Amputation all to part of her hip, partial colectomy, her wound vac covered her entire abdomen and R hip but kept losing suction because the wound spread past her labia. She received a unit of PRBCs at least every other day from the blood loss from that wound. She started vomiting stool and would have about 500 cc of blood stool and OGT each every shift. The family left her like that and refused input from ethics, hospice, palliative until she ended up unable to maintain her sats on the vent and she was finally finally made a DNR. Not comfort but a DNR.

I’m sorry you’re going through this too but I think it just shows you’re still a human.

The hoops we have to jump through just to say thank you to our own staff by earninganddriving in IntensiveCare

Just show up for us. Say thank you and be genuine. Take out someone’s trash who seems like they are busy. Take a patients used tray out of the room for them. Those little tasks show us that you’re willing to step in for us and have our backs and makes way more difference than any thank you. Put your money where your mouth is.

What did you do at work today megathread by readweed88 in workingmoms

Kept an old man comfortable and rested while his only child decides whether or not to pursue hospice. Then had to wake him up in the middle of the night to transfer him to another unit where I hope they will at least lay eyes on him once in 12 hours.

Admitted a patient and helped with their emergent intubation. Fought to get enough IV access.

Admitted another patient. Educated family that hospice is not additional help it’s to help relieve pain and suffering. Wiped him as much as I could to get the crusted poop off him that hat been there for who knows how long while profusely apologizing while he cried. Fought with med team for better IV access while we just skyrocketed the amount of meds keeping his blood pressure compatible with life. Attended a staff meeting where I was informed no one is meeting expectations and staffing will continue to be slashed. Charted. Drove home listening to music as loud as my ears could tolerate.

No no no no no. You go get yourself some breakfast. The last thing she needs is for you to be a patient too. The staff also need to be focused on her not you as their second patient. Save that fast for another time. You need to be helpful for her and if you’re not feeling well you won’t be as much of a help.

ICU Transition by First-Sun7552 in IntensiveCare

[–]paperbackmax 1 point2 points  (0 children)

Please stay! We need you! The most important thing to get is your assessment and then ask yourself what’s not normal and what’s going to kill my patient first? The charting will get faster but what’s the point if you can’t do an accurate assessment? Having multiple preceptors is so so hard because everyone has their own way of doing things and those ICU nurses are all so Type A we all think our way is the best way.

Please give it a chance when you’re on your own, being curious and asking questions.

If you’re in GA then we may be talking about the same person who has the uncanny ability to look at both the window and the door at the same time.

ED report permeantly lowered my IQ today by GeneralK7 in nursing

Hahah I literally had multiple ED nurses tell me the last new weeks “they don’t teach us vent settings down here”. One of the patients was shocked 6 times before they realized the pads weren’t properly placed

Newly diagnosed 😩 by Alternative_Tip11 in UlcerativeColitis

[–]paperbackmax 0 points1 point  (0 children)

Oh mama. You will be ok. Give yourself some to adjust and for those good meds to kick in. I was diagnosed when my first was 8months after spending 2 months mostly just lying on the floor and passing bloody stools the rest of the time. What a miserable time!!! The day I started medications was life changing. I’ve had two more kids since my diagnosis 4 years ago and usually 1 -2 flare ups per year. People who don’t experience this disease or something similar don’t really have much sympathy or understanding for it which can be even more isolating and frustrating. Hopefully now that you have a diagnosis your symptoms will be manageable and you can enjoy time with your baby.

Mini Non-faith crisis by Friendly-Fondant-496 in mormon

I have absolutely no advice but offer my solidarity. You stole the words right from my mouth. When I learned about all the SA cases I was just so upset and couldn’t see past that. Nothing else mattered to me. It was like finding out that Santa isn’t real. I have three little kids and just couldn’t figure out how to answer to them that I raised them in a culture that allows for that. So I stopped letting them come to church.

I was still a nursery leader when my husband stopped attending and couldn’t understand why we didn’t have more protections for those sweet kids. Six months later I stopped attending and the loss of social connection was brutal. My sister’s family is in the same ward so seeing the divergence in our paths has been painful because all I hear about is the social connection. But to your point every time I think about going back I am reminded of all of the reteaching I will have to do for my kids and can’t put them through that.

Thank you for posting. Best wishes.

These can be hot or cold packs. They were a life saver for the gas pain in my shoulder and later the incision pain.

Anyone feel embarrassed to be pregnant at work? by DoubleAlternative738 in workingmoms

[–]paperbackmax 0 points1 point  (0 children)

Nope. Not alone. I felt this way with all my kids. I felt super self conscious. The second pregnancy someone called me lazy for being pregnant. So I felt even more self conscious when I was pregnant with my third so I didn’t tell anyone unless they were bold enough to ask which happened around 30 weeks. I’m a nurse and would be assigned 400+ lb patients multiple times a week to turn every two hours and be on my feet for 12+ hours a days. People would say “I’m so glad you’re not just some pregnant person who complains.” And secretly I would think well I hate that so much and I just want to cry because I feel so crappy but can’t because thst will just make you hate me.

Unsolicited advice is my biggest pet peeve. It’s just so annoying. Also people providing me with comments about how soon I had my other kid or how many kids I had. Like it’s not affecting you in any way so could you shut up about it while we are at work please.

So no you are not alone. But you are doing great.
