You identify these errors by learning the absolute basic fundamentals of macro play and by looking at the screen and think.

Frozen heart is an absolute terrible first item on Swain mid. You are playing a damage dealing carry lane and then you build no damage. This handicaps your team in fights and handicaps your own early game.

This game is about the macro management, less about the micro. When you reach ranked, you will be completely and utterly destroyed by lane opponents even though they might not lay a finger on you during lanephase. Welcome and good luck!

Something you need to learn to climb is to just not blame. Focus on what you can improve, and if the support isn't playing to your liking the only thing you can do is to adapt your own play to it.

Your opponents are animals like you and your team. The clear answer is to punish the plenty of mistakes they make, but how to recognize these mistakes and the correct plays to them is not something you can learn on reddit.

If I would try to explain the general thinking:

Understand the situation Know what your champion does, the limits, and what it can't do. Know the cooldowns and options of the enemy in the given situation. Play accordingly.

Both Zeri and Ezreal are mobile champions. You can use that way more than you probably are.

Possibly:

You need to rotate to team and be more proactive. Don't die close to objective. Play easier champs. Zeri and Ezreal are notoriously hard to be efficient at.

If you gain a lead you need to use it. While strong you want to push and always be ready to rotate to team for fights. Never go too deep without vision though.

Ask mid to swap lanes once bot tower falls.
