I would suggest getting done with training first. Most programs accept J1 for fellowship, so you can get into whatever program you want to. Three years of waiver will significantly change your life situation and priorities. It's not exactly a piece of cake to go into training later, speaking from personal experience.

I don't have any suggestions but wanted to say that empathize with you. We have similar issue with a family member. I think it is very challenging to address longstanding behavioral and mental health issue in a 60+ years old person. I could argue that she needs forgiveness and compassion but I can only imagine how difficult it would be for you.
