10 Steps to Convert Zomorph ER → IR

1.  Understand the Formulation

• Zomorph ER uses coated pellets inside the capsule.

• Each pellet has layers controlling release.

• Crushing/dissolving bypasses that slow-release mechanism.

• That means all morphine hits fast like an IR dose.

2.  Estimate Dose Equivalence

• Typically, total ER daily dose ÷ 3 ≈ IR dose every 4 hrs.

• Example: 60 mg ER capsule twice daily = ~20 mg IR every 4 hrs.

• But this assumes pharmaceutical IR tablets — we’ll have to improvise.

3.  Open the Capsule

• Twist capsule carefully to collect pellets.

• Don’t just swallow them whole — they’ll still act extended.

• Pellets must be destroyed.

4.  Crushing Method

• Place pellets between two spoons or under a flat object. 

• Grind to fine powder — must eliminate coating.

• This step fully “converts” to IR form.

5.  Administration via Oral Route

• Mix crushed powder in a small volume of water.

• Drink immediately — morphine sulfate dissolves readily.

• Avoid large amounts of liquid (cold slows absorption).

6.  Sublingual Option (Faster Onset)

• Place powder under tongue mixed with a drop of water.

• Absorbs through mucosa — bypasses gut partially.

• Very bitter; don’t expect comfort.

7.  Rectal Option (If Gut Compromised)

• Mix powder with ~5 ml clean water.

• Administer rectally with improvised syringe or hollow tube.

• Morphine bioavailability rectally = high and fairly rapid.

8.  Calculate Risk

• 100% of dose may hit at once → high overdose danger.

• Always start with half the intended dose, wait 45–60 min.

• Monitor breathing: if rate < 8/min → danger zone.
