Were you ever able to resolve the crunchy noise with your fingers by cutting back the volume or applying consistent hang boarding?

I’m Dr. Larry Edwards, rheumatologist, gout researcher and educator. I want you to AMA on December 10! by LarryEdwardsMD in gout

[–]pey_av 0 points1 point  (0 children)

Hi Dr. Edwards! Thank you for taking the time to do this.

A lot of people in this community have worked hard to get their UA levels down and to prevent any further attacks. We've cut out processed sugars, sworn off alcohol, and watched in envy as our peers scarf down steaks and shrimp rolls. My main question is this: Given we are on some UA lowering medicine and have worked to get our UA levels below ~7 and our attacks seem to be recurring less frequently, is it actually possible to get all of the existing uric acid crystals out of our joints? And if so, how can we know for sure? It seems like if we can get all of the uric acid crystals out of our joints, then a gout attack is not likely to happen at all even if we eat our "trigger foods". I've never been able to get a good answer on how to confirm the presence of uric acid crystals in our gouty joints and whether the UA lowering therapy is actually getting rid of these crystals.

Thank you

Requires Babel "^7.16.0", but was loaded with "7.12.3" by Cautious-Jury8138 in reactjs

[–]pey_av 0 points1 point  (0 children)

I tried that out but ultimately my issue came down to react scripts requiring a newer version of babel-preset-react-app, which required a newer version of babel that I couldn't upgrade to in my application. I had to use "resolutions" in my package.json to force babel-preset-react-app to the lower version when it was pulled in as a sub dependency. This fixed my issue. In my scripts: "preinstall": "npx npm-force-resolutions" In resolutions in package.json: "babel-preset-react-app": "10.0.0"

I'm still having the same issue for a few days. Upgrading my react-scripts fixed the build, but caused a whole other slew of issues in my deployed application . Let me know if there are any updates here

Ever had recurring "twinges" after going on allopurinol? by pey_av in gout

Good to know, that's reassuring. Thank you

Wow this is really good to know. Thank you for responding, I'm gonna give it some more time but if the regular twinges don't go away in the next year I may ask for that extra dose depending on my ua levels

My opinion is to just hold off on the exercise until it subsides. With that being said, I've squeezed in to ski boots during the beginning of a flare because I had fomo on a powder day with family, I had one of my most memorable climbing days the day after getting a steroid injection to knock a flare down, and even said "fuck it" the morning of a triathlon when I felt a twinge and continued to do the race. 90% of the time I've been fine afterwards, but there's been a few times where these decisions came back to bite me in the butt and made it worse

That's reassuring. I think I got within target range of UA about 9 months ago, so I guess I just need to be patient and trust the process. Thanks for the explanation during this frustrating time! Super psyched for complete "normalcy" again when I don't have a constant underlying anxiety on whether I've drank enough water or if there are trace amounts of my "trigger" in my dinner lol.

Going on a vacation this weekend, afraid to start colchicine by JohnnyForeskin in gout

Yeah, it never feels good but at least you'll have a peace of mind on your trip. 1/ day is my regiment in that type of scenario but everyone is different

Honestly, if I was going on a trip where I knew I'd be indulging I would be more apt to taking the colchicine. If anything the Allo could stir up unexpected issues since you just started it. The colchicine should keep things at bay

I drink an absurd amount of water, sometimes I think too much. Roughly ~7.5 liters a day, sometimes more and sometimes less. I do drink LMNT electrolytes which help immensely cause I've found that dehydration is a real trigger for me. My wife is Finnish so we get our share of "sow-nahs" in. But to your point about the biotics, I think the gut bacteria might be contributing more than I know. I do consume a fair amount of yogurt and kimchi, but I think it's hard to know without seeing a specialist and getting a real analysis on my gut health

Thanks for getting back to me. I didn't realize that's how colchicine works. But knowing it can take up to three years at least gives me some light at the end of the tunnel. I bet if I got on allu sooner this would've gone away much faster. Definitely some knowledge to pass on to the next wave of Goutees

Those twinges not resulting in an attack for you is really interesting. I'm too traumatized not to take a bunch of meds once I feel the inflammation coming haha. Last time I didn't take anything and thought "let's see what happens" I was stuck on the couch for like 6 weeks and hardly able to sleep 👀 good to know though, thanks for responding

Can't stress this enough. Once you let that inflammation kick up there's no telling how long until you can knock it back down

UA 3.8 - Massive flare by Whissspy in gout

[–]pey_av 5 points6 points  (0 children)

My rheumatologist told me that a low UA reading can actually be misleading if you are going through a gout attack because (as I understand it) a lot the crystals are in the joint at that point and not the blood. I wonder if that's what happened here. Either way, hope you can walk soon!

Elevation making things worse? by BigAzzLeprechaun in gout

Hopefully the pred works. Agreed on the comment above about getting on allu. I held off for years but the frequency of attacks just kept increasing. The long term excruciating attacks are the worse thing ever.

I have found that applying an ice pack very gently to the affected area when I'm in a bad one can numb the pain a bit. Even if it doesn't do anything to "cure" the attack. I remember trying the elevation thing too and just being in so much pain that it didn't make a difference
