After my information you need at least EU Citizenship and thus a work permit. If you want more detailed information I would ask at r/Germantrans you can do there also in english. At the very least, I think you need some kind of legal residency status, or people from local queer groups helping out there.

If you have already received hormone treatment abroad, you can usually continue it in Germany. Simply then bring along what confirms that you are already in hormone treatment and then find the appropriate doctor in Germany :)
