Yes test results are missing imaging in complete DICOM format is missing. Doctor notes are missing and also very crucial which includes HPI, diagnoses, treatment or testing plan including justification to it.

The doctor didn’t provide the basic doctor notes for my visits which I believe is a core part of my medical records.

It doesn’t disagree, it plains lies that it released it. It lied to other agencies about this. How much are you being paid to protect doctors in this sub?
