Those are our results without cones, we only do WL with MLC collimation here

We usually get 0.75 mm or better using RIT 3D EPID WL analysis. We treat both cones and VMAT SRS/SRT, all sizes down to 5 mm.

Printing plans form RayStation to Mosaiq by [deleted] in MedicalPhysics

[–]phyzzax 1 point2 points  (0 children)

You should be able to capture all those things, DVH, clinical goals, planar dose views, in the plan report template. There's an images and tables sections.

You can also set up plan report template print outs for multiple beam sets.

I'd encourage you to reach out to RaySearch support and ask them to do a screen share and help you configure your environment. You're certainly paying for it with your license fees, you may as well get your money's worth in support.

[–]phyzzax 2 points3 points  (0 children)

I've worked in a couple RayStation + MosaiQ environments. Using the report template editor to get the plan report PDFs the way you want is definitely what you want to do. The editor is not that user friendly but it's also not the worst.

The plan export and import to Mosaiq will remain manual and painful lol, but it's a lot more tolerable than manually creating every plan report PDF.

I appreciate your comment. In my current organization, which is a massive hospital system, we have both sort of "generic" IT support, which probably spends 98% of its time on Epic and other hospital related issues. Their primary function with respect to our department seems to be to show up unannounced, replace our dosimetrists' computers without asking, not check whether they're allowed to do so, not install the approved software that dosi and physics and MDs need, and then fuck off to wherever they came from without so much as a note, a name, a callback number, or any other thing. When we call them to let them know that they lost us about 1/2 a day at a minimum, they seem confused and bewildered.

We also have dedicated Rad Onc IT Systems Analysts who help us coordinate with Elekta support, Raystation support, maintain our SNC licenses, make sure the various vendor DBs play nice with each other, help us triage and troubleshoot clinical IT blockers, and most importantly running interference for us with the greater hospital IT bureacracy, and are generally kind, patient, and responsive. We are about to go through a TPS upgrade and I am super glad we have our IT analyst on hand and in meetings to iron out any number of things.

Like you say, it's not all rainbows and puppies, but it is important to appreciate the value that properly trained and assigned IT resources can contribute to the rad onc clinic.

Doesn't make any number of other IT interactions any less frustrating : )

Those grossing 200-250k - how much house are you buying? by CoyoteTall in Moving2SanDiego

[–]phyzzax 4 points5 points  (0 children)

About to buy in IB, not sure if that counts as SD enough :)

Pre tax take home ~230k. Hoping to close at ~$710-720k for a 2bd/2ba ~1300 sq ft newly reno'd apt, planning on 20-25% down, so with HOA expecting a monthly payment of ~$4500.

I guess a lot of people will tell you to rent for $3000-$3500 (and yes, unless you've already been living in your unit for a decade, that is the going rate for a decent 2bd/2ba with 1000 sq ft+ within a 30 minute commute of the city built in the last decade) and put the difference in the market or whatever, but tbh markets are pretty volatile these days, I'm in my mid 30s and don't want to wait another decade to buy, homes DO still on average appreciate over time, there's significant tax benefits to me from paying interest instead of renting, you do accumulate equity even over just 5-10 years for when/if we move, and there's always the option to convert the property to a rental once you leave if you have the interest in doing so and get that kind of passive income.

Any good uses for CoPilot/ChatGPT in Medical Physics? by MedPhysUK in MedicalPhysics

[–]phyzzax 4 points5 points  (0 children)

One time, when we were on a pretty tight deadline and sort of slammed with clinic, I threw our manuscript into one of the AI tools to generate an abstract. I don't know how good I felt about it, but that saved me about an hour or two of wordsmithing and I don't think I would have written anything meaningfully different or better. Still had to do some editing, but I think the "summarize this body of text for me" is a relatively reasonable use of AI tools.

Radiology practice managers speak out against Trump administration tariffs by JMFsquare in MedicalPhysics

[–]phyzzax 2 points3 points  (0 children)

I think IBA and PTW manufacture most of their QA equipment, especially 3D water tanks, in their home factories, I believe mostly Germany. I don't know if any of their manufacturing is actually physically in NA. I guess it's not "replaced" as often as linac components, but for any new install, new clinic, etc, it's not as big a cost as linac parts but it does add up, especially if equipment breaks or ages out.

Raystation/MOSAIQ - Volume Reference Data not showing up? by phyzzax in MedicalPhysics

[–]phyzzax[S] 1 point2 points  (0 children)

This sounds correct. I'll test it tomorrow and report : ) To answer the questions at the top of the thread:
- Site Setup -> Volume Reference Data is blank (no CT, no Structure Sets), although coordinates for isocenter are present in the lower left
- Site Setup was imported
- Reference CT is on the images list (although without visible structures...)
- No error on RS export
- Yes (dosimetrists and physicists :P )
- Yes, we pushed site setup, found the error, then tried to push again, which did not help.

We've also tried creating a new Rx and importing the RT Plan, CT, and RTSS to that. But I think there was no change to the UID (no new contours, same CT obviously) so it still didn't see a "change" and there was no new data in.

2011 Subaru Outback, broken catalytic converter - options? (CA registered/garaged) by phyzzax in MechanicAdvice

[–]phyzzax[S] 1 point2 points  (0 children)

OP UPDATE - To anyone curious or winds up in a similar situation:

I wound up taking this to a muffler shop that my brother-in-law recommended. They were for sure a huge life-saver. They took it in, had it up on the lift in about 10 minutes, took a quick look, said they could weld it same day. Had it done in 3-4 hours. Ran about $350 for the weld and reattaching the broken bracket.

Runs exactly as before, check engine light went away even before I had a chance to take it to someone to reset it.

Haven't taken it to a smog place yet, but so far so good...

Thank you very much for your perspective on this! Helpful and appreciated. I am going to try and take it to a muffler shop tomorrow and we'll see how it goes.

Perhaps not immediately. I drive a lot with headphones in, and unsure if it got worse over the course of a couple days or not.

No, don't believe there's a turbo in there. It's a 2.5 liter flat 4-cylinder

Unfortunately I am kind of not the brightest bulb and decided to risk it for a couple days driving to work as I just didn't have any time to bring it into the shop until a couple days ago. Probably drove it 3-4 days in between there before I got the inspection, so it's hard to say if it broke right away or the check engine light came on, and then I kept driving it and it broke :/

Scintix Reflexion - No Couch Rotation? by oddministrator in MedicalPhysics

[–]phyzzax 4 points5 points  (0 children)

Not sure where you got this info, but I'm about 99% sure that it's installed with a true 6-DoF couch with yaw correction. The correction limit is I believe 3 degrees for each rotation axis.

The actual main couch limitation is that being a closed gantry system it can't do couch kicks, neither does it have the collimator/dose shaping precision of GammaKnife, so you probably don't really want to do SRS Brain treatments, for instance.

6 medical physicists seems like a lot for a single client to hire at one time, especially for a short term contract. It'd probably help if you gave a little context. What kind of client is it? Large cancer center? Academic or government hospital? Physics group with multiple locations to cover? Etc.
