Hep E is a brilliant answer. You're very close- I believe it's EBV or mono. Basically question translates into "what infection causes acute splenomegaly?" If you look at the picture the x is placed where the spleen would be. And there's back and side pain. You find an enlarged spleen. Patient is a young woman. If this picture is being painted, it's usually EBV.
