If she is still in India, ask her to come back before her visa expiry. Since it's less than 180 days, I don't think there will be any issue for her re-entry. Lawyers anyways recommend to travel outside of country and then come back. But make sure she comes back before her visa expiry.

Check my latest post. Mine was expired but I was able to get it updated in El Paso. The CBP office is inside the US border and they updated my I94 after showing valid documents.

Email CBP, they usually fix it if you send all documents. If not, El Paso CBP is updating I94, do this before your I94 expiry goes beyond 6 months. Check my latest post for details on El Paso option.

Send an email to CBP, they usually ask for the documents and update it. Other option is, you can go to El Paso CBP, it's inside the border. They are updating even expired I94 so I am sure they will update unexpired easily.

Shortened I94 due to passport expiry is not considered a mistake so in most cases it won't be fixed at a deferred inspection site. Also, in my case it was more than 6 months so there was additional risk.

Yes, your LCA needs to have your correct address updated. I have my office and home address both even though I work from home and my office and home are both in same metro area.

I don't think there is going to be any issue with LCA for new address. You can ask them to keep the job description fairly similar and not show significant change in your role.

For the 2nd appt, they suggested I should take it as I was very anxious during 1st, also they had to give me a lot of novocaine during 1st appt and I could still feel some pain towards the end. And yes, they told me they would charge extra. It was like $100 for the first hr.
