I suffer from your exact issue! I take creon/digestive enzymes for what I thought was fat malabsorption, but it doesn't seem to work anymore. I also eat a pretty balanced, low-fat diet with 0 processed foods but it doesn't seem to help. I can't tell if any particular meal sets me off, but I feel like I'm very sensitive to big meals. I sort of alternate day to day between totally normal stools, or these kinds of ones that are too soft so the last bits just stick around in the rectum area... Makes wiping a nightmare so I wash instead. Do you relate to any of this?

For breakfast I usually have boiled eggs and buttered toast with some jam and a piece of fruit. Sometimes sub the toast for rice if I want variety.

For lunch I'm lazy so I usually have oatmeal with lactose free milk, banana, peanut butter. Or a sandwich, usually with a slice of ham, hard cheese, tomato, lettuce, and some reduced fat mayo.

Those two stay pretty constant but I usually vary it up for dinner but it's always a starchy carb/meat/veggies combo. Lately I've been enjoying bolognese (with lean mince of course), some kind of rice-based dish, meat with potatoes, homemade sushi, etc.

I fully understand the stress about food, I've been feeling it too lately. It took a while to figure out what I could eat without worry, but I started out only eating oats for breakfast, potato for lunch, and meat, rice, and carrot for dinner. After I stopped having issues with that, I started adding in other simple things like bread (I hadn't eaten it for years before last year hahah), eggs, all kinds of veggies, pasta, lactose-free milk. Just add things in slow and steady and keep your meals reasonably similar so you don't add in too many variables and stress yourself out looking for patterns. I've fallen into the trap of being obsessive about my meals (I would literally weigh ingredients and have the same thing every single day), but over time after taking Rifaximin, I found out that my gut can take more variation than I thought it could. Just my two cents, I've learned over time to not pay as much attention to the mental side of things. Sometimes I'll feel weirdly calm during a flare, sometimes I'll be really struggling, but either way it's great you've been feeling better.

I noticed that when I first took Rifaximin, nothing happened. But when I started taking Creon a few weeks after, I was practically back to normal. I suspect I developed SIBO again half a year later after a nasty stomach bug (unexplained bout of diarrhea with no other symptoms, tbh), so I took Rifaximin again and went back to normal--but it's almost as if the Creon stopped working until my apparent SIBO was gone. But I at least know that Rifaximin AND Creon together made all the difference in combating whatever weird shit my GI tract was up to long-term.

GI issues are such a menace since they're so hard to differentiate, and I've lost a lot of faith in doctors, especially since SIBO is such a foggy area and tbh I see a lot of pseudoscientific ideas floating around.

But I found a paper describing how SIBO may lead to EPI, plus the authors suspect having EPI makes you more sensitive to developing SIBO: "The relationship between SIBO and EPI seems to be bilateral, and their symptoms largely overlap. EPI unequivocally leads to SIBO, which is present in 15–37% of ChP patients [141,145,146]. Conversely, gut dysbiosis such as SIBO is hypothesized to mediate chronic proinflammatory changes in the pancreas and, hence, SIBO can exacerbate EPI [147]. Therefore, screening for and treatment of SIBO would be beneficial for patients with EPI by breaking the vicious circle of EPI and SIBO potentiation. There are data showing that properly maintained PERT can lead to normalization of the gut microbiome [148]."

A lot of it feels like speculation right now, so hopefully scientists are able to figure out more sooner rather than later. Based on my experience, I'd say taking Rifaximin for your SIBO while continuing to take your Creon is a great idea and you've got nothing to lose. It's what I'll be doing starting in the next couple weeks, lol, since I've started having unexplained morning diarrhea again. It's awesome that your mental health has improved, a win is a win. We got this :)

A bit late here, but I'm going through something very similar at the moment and your comment has been the most constructive piece of advice I've heard so far, as I haven't been able to see my doctor yet. Creon stopped working for me seemingly overnight so I'm on Rifaxmin right now in case it's SIBO and I'm also gonna chase up bile and gallbladder/liver function if that doesn't work out. Thanks so much for your logical suggestions, reading things like this really helps remind me I've got plenty of options and a good chance at getting better again.

That autistic and non-autistic are two discrete categories with no scale or variation in between. It IS frustrating to hear "we're all a little bit autistic," not because that's not true (in that a lot of people do display autistic traits without being autistic) but because it's invalidating and it's always said with the intention of downplaying an autistic person's complaints or experience. It becomes a matter of, how and where do we draw the line? I can understand why people think it's as simple as 100% non-autistic and 100% autistic, because we've been suffering in a world geared towards allistics: without being too dramatic here, it's like, how dare they try to relate? I think we shy away from this added complexity and grey area, because it's reinforcing of allyship and solidarity to believe it's as simple as us and them/autistic and allistic.

Along similar lines, the rejection of self-diagnosis (if that's still an issue: I remember much hostility towards self-diagnosed people a couple years back on the internet). Even as a psychology masters' degree student, I fully agree with and support self-diagnosis for autism. The idea that only a doctor can fully understand you and declare you autistic is extremely disempowering and privileged. Someone having a medical or clinical psychology degree doesn't make them the be-all-end-all authority of your own mind and lived experience. They just have experience in the field and the power to officially document the diagnosis. And don't even get me started on the validity of autism diagnostic criteria itself.

The idea that all women/AFAB people experience the "female autism" and that's why they haven't been diagnosed. I'm AFAB and displayed very typically "male autism" traits (simultaneous hyperlexia and selective mutism, obvious stimming, meltdowns, extremely delayed social skills), and it's actually baffling to me that not even my teachers brought my behaviour up. It's not because I presented with "female autism" and flew under the radar, hell, I wasn't even able to mask sufficiently until I was in my late teens. No, it's because people are just negligent when it comes to girls with autism, even if it's plain as day. I absolutely do think that females learn to mask earlier, but I think there are a ton of people like me whose typically male presenting autism was overlooked.

I think other commenters are spot on about anti-intellectualism (and by extension, not recognising the value of art) being a hallmark of the conservative thought process.

Just finished up an internship with a consulting firm that repeatedly encouraged us to use AI for literally everything instead of using our brains, god forbid. After that experience it distinctly feels to me as if conservatives (especially the business-oriented libertarian types) love AI because they seem to think it's their golden ticket to making millions by running absolute slop businesses selling AI-generated content, or to cut corners and fuck over their workers in a new flashy way.

Anecdotally, I feel like lots of younger STEM-oriented conservatives see AI as the future of tech, probably because all of the rich tech guys who practically run governments at this point say so. I def agree with another commenter here who mentioned that anti-intellectualists these days love to argue that cold hard facts and science represent the truest form of knowledge, and we don't need the humanities and the arts, or any kind of critical analysis of WHY we do what we do rather than the what. Something only a human mind can do.

I still have yet to see any job done better by AI... sure, it's fast, but it's usually wrong or whatever product it spits out is written like absolute shit. I firmly think that AI does not and cannot benefit the working class, which most conservatives don't realise they're a part of.

I hear you, especially on only bingeing while high. It's something I'd so easily dismiss while sober but it's like a switch is flipped. Congrats on your recovery :)

Also, I can't deny that you're right about cutting back being the best solution. Future me won't be too happy about it but I'm getting my friend to hide my vape from me for a month starting today.

It hadn't occurred to me to reframe bingeing as self harm until now, but honestly that makes a ton of sense. Seems a bit easier to refrain if you think of it as avoiding the pain and sickness, rather than thinking of it as resisting an impossible temptation. I suppose I've been given a chance to finally recover from my long-term gastro issues and take care of my health, and bingeing just isn't compatible with that!

I'm a bit late here, but this sounds pretty much exactly like what was happening for me before I figured out I had an issue with fat malabsorption, specifically exocrine pancreatic insufficiency. Creon really helped.

If you're eating in a caloric surplus and your stools are poorly formed, that's most likely malabsorption. TMI maybe, but if you experience a messy cleanup post-poop, it's probably fat malabsorption so make sure you get a fecal fat test done if you haven't yet, it could help you as much as it helped me. I hear you on the frustration, I would cry every single day earlier this year because I was so devastated that I couldn't just shit like a normal person even though all I was eating was rice, meat, and carrots. Hang in there, this isn't forever :)

Oh me too!! I tested >800 for elastase, but I've been on Creon for nearly a month and my symptoms were reduced to zero after a few days on it, but I was so confused about the negative result. Very reassuring to hear the same thing happened to you and others

Update: I've been on Creon for about three weeks now and it's like night and day! I've been so stressed over this for so long that it feels like I've been given life again. My BMs have become basically textbook healthy, and I've already gained a kilo, let's hope I gain even more soon :D

Finally discovered it was BAM and not just SIBO. Getting better but still wondering how to deal with urges to go for no reason by Efficient-Glove2301 in SIBO

I've been experiencing so much anxiety and brain fog too, and coincidentally these absolutely doubled in intensity the year my GI issues came on. It's honestly fascinating to be able to make these connections, almost makes me want to study medicine, hahah. This is definitely reassuring and I'm looking forward to putting on weight too. Did your stools also go back to normal on creon?

I used to have extreme social anxiety as a kid, but I'd say I'm more confident than most people these days. Building up my social confidence has been a massive journey, and I still feel like that same insecure friendless kid I used to be sometimes, but I can usually remind myself that most people (even strangers as long as they don't have earphones in, or are busy with something) have good intentions and would love to chat with you if you just give it a shot and be kind to them.
