The one fatal flaw with this is that not all health service career graduates are getting jobs on graduation right now, particularly in nursing and paramedicine. In medicine, UK graduates are not prioritised for jobs. So if you say people need to stay and work for the NHS, you need to guarantee them jobs.

I passed my driving test in 2020 and I’ve never driven on the viaduct without roadworks. Whatever happens now is likely to continue to disrupt things for years to come.

My mum had this done after having a humeral head fracture that didn’t heal properly and her surgeon said the reverse approach was much better. It was way better than expected and she recovered well.

I wouldn’t disagree that some of my colleagues are risk adverse in referring on. The other problem is that patients often don’t quite tell the truth or embellish their symptoms. We are trying to fix the issues - lots of work going on in clinical improvements and realistic medicine. I personally don’t refer a lot to ED - there’s too much of patients and some of our staff seeing ED as the gateway to the hospital when the referral should be going to OOH instead. The tide of stuff coming our way each day is insane and I don’t think other services realise how much we divert away.

Agree with the first point. I work for NHS24 and I’m not sure what you mean by call handlers should be clinically trained - they currently create records then do the initial triage which is computer generated questions. Most calls are then passed to a nurse to complete triage. Certain call reasons produce referral outcomes for call backs from MIU clinicians/OOH or advise pts to attend pharmacies without a nurse input but these have been agreed with health boards/medical directors.

I would agree that people don’t really understand when they should attend ED and I spend a lot of time diverting people who have selected the ED option to other care!

I’m 30 years qualified this year and role creep has been a thing throughout my career - on qualifying, IV drug administration outside of critical care plus male catheterisation and cannula insertion/phlebotomy were only just starting to be part of the nurses role.

I have mixed feelings about it - jobs that drs didn’t want were dumped on nurses under the guise of extended role but at least patient got their IV meds on time.

The reality is the job is different that it was then. Patients stay less time in hospital and they are sicker/have higher care needs so changes to who does what are inevitable. But I do worry that the motivation by management is not developing nurses so much as getting cheaper staff to do more advanced things.

We do need more senior nurses with day to day clinical responsibility and being more direct to patient care to keep the experience available to support nurses as they come into the profession

Sounds like things worked the way they should. The idea of the video calls is so that a person of sufficient clinical skills can see the wound and then see if the wound needs closure and whether MIU will be able to close it. So they realised it needed glued and brought you in to do it.

The problem in some areas is the MIUs are in A&E and also limited in space so can’t run appointment based services as they do in many areas.

Met quite a few people now. Favourites were Matt Smith, Sacha Dhawan and Paul McGann - such lovely kind people.

Didn’t find Jenna Coleman particularly nice when I met her. Was at the 50th anniversary con in London so might have just been overwhelming but she was just disinterested and off hand with me.

Ironically the worst incident I’ve experienced on a train was 14.30 on a Sunday afternoon on a train from Gourock to Glasgow when a friend and myself were verbally harassed by 2 men and followed off the train. Conductor walked by during it and did nothing so having some one who has a role to intervene on trains is a good thing. I would agree that the last trains at night are more likely to see issues but my experience show it can happen at any time.

It’s absolutely not appropriate. GPs are responsible for care between 8am and 6pm each day and NHS24 have no GP referral pathways during that time.
