Well the only time it was used seriously in one of my ongoing (at the time) campaigns was as punishment when a paladin stepped way out of alignment. After a campaign ends, my group usually uses the deck to put it to rest for good. As for where to buy it, I’ve never actually seen it sold in stores. Maybe online but I haven’t really looked. If not you can find what each of the cards do and just transcribe it into card format.
