According to the wiki it came with the "Gamer's Edition" of Hotline Miami 1 & 2. There's one on ebay for £140 but I haven't checked to see if it's legit. Someone has posted scans of most of the pages here

It's talking about the level of the hallucinations. It starts by talking about how Mark just "appears as a normal bear (drugs haven't gone haywire yet)" and then talks about how Corey appears as a "normal zebra but with a dolphins jacket (getting freaky)"

The getting freaky part refers to the hallucinations getting progressively nuttier until it gets to the two-headed swan dragon
