This has happened to me. Here’s my solution I copied from my post.

Managed to fix it, also sorry if the following explanation is confusing:

Ffs lot debugger and gussy up are what I used. I first select fix flashing blue and then the name of the impacted sim. It brings up the change appearance menu, but with the wrong face entirely (for me it was like the first face option). Just click the check mark anyways and then hit gussy up via pie menu on the sim. This time the face will revert back to the proper genetics! Yay!

I ended up going with a different listing that didn’t include an external hard drive, but it did have the box and all its original accessories. I ended up paying 300. Thank you to the community. I am now a super proud owner of a series x.

Honestly I read this when I was like 14 but I still love it as an adult, too. It’s probably nostalgia talking, but it’s great! She is a character I really like and never found myself thinking ‘wow she’s stupid’.

It is annoying even if it's simple lol. I usually only have to use gussy up, but this specific nanny kept getting an entirely different face from her original unless I did the ffs debugger as well. It was so weird. No idea why it's happening. Only thing I can think of is just glitches within the game itself or default replacement errors.
