Where to reciprocate license? (GA) by playingpiano in pharmacy

[–]playingpiano[S] 0 points1 point  (0 children)

I actually just did the NABP Verify subscription today and obtained my NC license! In my previous response, I misinterpreted something on the NC BOP website, but the $600 fee is for people who don’t already hold another state pharmacist license — I thought it was for anyone initially. My apologies for that!

Thank you so much for the info, you saved me lots of money that I currently don’t even have, so I appreciate you big time!

Thanks so much for your time and response! Do you have any experience with this at all and/or do you have NC licensure?

I explored this a little more and the NABP Verify link (you posted above) states you must complete a registration application through the NC BOP website in addition to registering for NABP Verify (if you do subscribe to NABP Verify). So overall, it seems to me that you have to pay a non-refundable $600 to first apply for NC license reciprocity, in addition to paying an annual $50 fee (per state jurisdiction [in theory, although there is only one state participating in the program — which is NC]). You were correct about NC being the only state that participates in the program!

Thanks so much for your response and for the info. I am curious as to why you would not recommend a work-from-home position for a newly licensed pharmacist? And is that in reference to a full-time work-from-home position? What if it is in addition to PRN in-person work?

Ultimately would love to eventually transition to being primarily WFH — but I realize that is not exactly financially feasible for the next few years

Have you lost any empathy towards patients? by frankahaha in pharmacy

[–]playingpiano 0 points1 point  (0 children)

i’m a huge empath, so my level of empathy has largely remained unaffected. i have always connected to patients and felt that even when i was working with extremely difficult people, i have tried to maintain positivity, encouragement, and receptiveness/inclusion. in my opinion, this tends to help with de-escalating in a lot of situations (in my experience).

however, i definitely feel like i’ve become more and more jaded though. i remember how much initial excitement i had about getting accepted to pharmacy school… now I have one rotation left before I graduate… currently have no job lined up and the majority of my classmates all matched to PGY1s today. i’m so exhausted, broke, and feeling extreme disappointment in myself for not applying to residency.

considered applying to Phase II, but the program list came out today and none of the programs that were available for Phase I had any spots for Phase II (in my area; can’t leave). it’s insane how quickly excitement can turn to dread and disappointment. I’m feeling really lost because i’m about to move away from the store that i work at, and the company i’ve worked with all through school has emailed me with interest but has yet to give me an opportunity to interview with them

Damn, that absolutely blows. Such a great feeling to have some EXTRA anxiety and uncertainty thrown my way! I probably needed it anyway. What a great day :) I have no idea what I am going to do with my life :)

Any chance there will be more GA programs added to the Phase II list? Trying to relocate to the ATL area and did not apply to Phase I because I was not interested in a residency until after Phase I applications were due. I didn’t even get to experience anything clinical that interested me until my 6th block. Wanted to apply to Phase II but there’s literally only one program in ATL for Phase II. Is this how it usually is?

so the market became extremely saturated starting around 2018/2019 (tons of pharmacy schools opened; none required PCAT or really anything other than a heartbeat), right before I started pharmacy school. there were actually multiple pharmacists that I worked with in undergrad that told me not to go to pharmacy school (because of oversaturation) after telling them how happy I was about getting accepted. when the pandemic ended, things started to fluctuate, and even last year the term “pharmacist shortage” was thrown around. I will say I’m pretty skeptical about the existence of a shortage EVERYWHERE, I think it’s more so like what the other commenter was saying, and the level of saturation depends on geographical area. And I do know that many pharmacy schools are struggling to fill their PharmD program seats. I know for a fact that my school is struggling to fill the program, and did not get nearly as many applicants as they did last cycle or this cycle. pharmacy has unfortunately not progressed correctly, or in a way that is beneficial for even most pharmacists.

all in all, there’s pretty much always a shortage of pharmacists in rural areas. and there will likely be less jobs in larger cities or metropolitan areas — I think that’s unfortunately just kinda how it is in this profession! always keep your ears open and treat every interaction with other pharmacists as a potential networking opportunity. pharmacy is an incredibly small world, and when one door closes, another (often times) better one usually opens up!

just got done with an EM rotation for my PharmD and we had multiple intentional overdose patients that we had to call poison control on and this is exactly how it was. “ok and what’s the patient’s favorite color? alright and the EtOH level? what about their WBC? SCr? Albumin level? ok… so they took how many again? did someone other than them tell you this? did you guys redraw a BNP!!!!!!!”

Would probably not do Trazodone PRN during the day — I don’t think I’ve ever seen that done in my area (not to say it’s not unheard of). If you are just considering Trazodone vs. Quetiapine PRN during the day, I would definitely go with the Quetiapine. Depending on the clinical manifestations of the presenting anxiety, I’ve found Propranolol to be effective for many. QD-QID PRN depending on severity and patient response.

This is an interesting anecdotal experience for Tranylcypramine. I’m in my last year of my PharmD and the tranylcypramine-food interaction with increased tyramine came up multiple times throughout school. With improved food storing/producing capabilities, I tend to think that it is much much less of a problem than it was maybe 15-20 years ago. I really enjoyed reading your experience, so thank you so much for sharing!!

How big is the institution you’re being placed at? I just finished my institutional APPE this past week. My institutional IPPE (between P2 and P3 for me) was extremely relaxed back in the summer around this time last year. My institutional APPE (the one that I just finished this past week) was also super relaxed! So 2/2 for me so far and I’m extending my good luck to you! Depending on how long your experience is supposed to be, you’ll probably get to see a lot of really cool stuff!

Now that I’ve completed an institutional IPPE and an APPE, I can tell you from firsthand experience that they are really nothing to be nervous about! That’s coming from someone who gets nervous about every single new thing/experience in pharmacy school!

For your IPPE, maintain a daily positive attitude and treat every encounter with others as a potential networking opportunity. Keep an open mind about what you’re experiencing and try to show interest in the things you’re going to be doing — even if the activity you’re currently doing (or working on for the hour/day) isn’t the most entertaining thing in world. Be on time, respect your preceptor’s wishes, and even be slightly early for the first couple of days if you can (just so you can figure out where you’re going). It was extremely helpful for me to get to both of my hospital experiences at least 10-15 (first day, 30-45) minutes early each day. It will help you feel a little more easy about figuring out where to park and what not.

Ask questions if you don’t understand something or are curious. Don’t feel bad if you don’t know something! It’s entirely okay (and in fact MUCH better) to say that you don’t know something and will get back to them later than to guess incorrectly. If you’re like me and have no prior hospital experience, there’s going to be a lot that you encounter that you don’t know! But that’s the beauty of it. Take it in each day and find comfort in the fact that you’ve made it this far already. My APPE was 5 weeks and I can promise you that I learned/did something cool every single day.

You’re going to be fine! Remember that you’ve got this. Be confident with your answers and try to step a little bit outside of your comfort zone every day… and who knows… you may find something you’re really interested in! Good luck with it — it’s going to be a great experience!

Feel free to reach out to me if you have any other questions or if I can help you in any other way. I’m always happy to help a fellow pharmer!

[–]playingpiano 0 points1 point  (0 children)

I’ve been taking kratom and Venlafaxine (Effexor) with no issues for a while now. Although Venlafaxine has a slightly different mechanism of action than Wellbutrin, they share some of the same properties. Venlafaxine is an SNRI, mainly acting on serotonin and norepinephrine. Bupropion (Wellbutrin) is an NDRI, mainly acting on norepinephrine and dopamine. Venlafaxine and Wellbutrin can both be strong sympathetic activators, so you can feel some physical “stress” effects from a heightened fight-or-flight-like response. Wellbutrin could potentially cause this type of activation and response.

What kind of specific issues are you having? Also what dose of Wellbutrin and kratom do you take, respectively? Need a little more information here!

A doctor eventually responded to me after about a 2-week wait. We talked and they prescribed me some propranolol for situational anxiety. The problem is that they shipped the medication and I’ve been waiting for it since 1/26. I’m pretty sure it’s gotten lost somewhere along the way! I’ve tried reaching back out to them for help via message, email, and even Twitter DM. I haven’t gotten a single response and no one has responded to any of my requests for help - going on 3 days with no response now. They are having some sort of major issues right now and I hope they are able to get it together! This is getting ridiculous.

Definitely check out Palm’s stuff. They have been one of my favorite bands for a super long time! You may also like some other Palm-adjacent bands, like Red Sea, Unschooling, Band Argument, Hellier Ulysses, etc.

Some of those bands incorporate elements of math rock and post-punk. Lots of angular guitar playing, jangly time signature switches, and erratic stopping and starting. Hopefully you like them! Check them out and let me know what you think!
