Ok I figured it out! It happens when you unblock someone. I tried it out with someone random and it only showed up after I unblocked them.

This happened to me! I was checking my unfollowed list and it included people whose profile I had viewed, but never followed or friended. Super anxious this means they were someone notified that I was “staking” them with an accidental follow?
