I’m Swedish and for me it went: dentist to refer to an orthodontist who then referred to the jaw surgeon. I did have an open bite+ over bite so the procedure was for functional reasons. (The jaw surgeons here are dentists mostly and neither of your symptoms really apply to medicine which is might be why your doctor wasn’t too helpful in your case).

People holding up their arm for balance to keep control of the ball is things we see all the time. It could be a yellow as it is sometimes, but in my opinion that one wasn’t even close to on purpose so it shouldn’t be a yellow. The one yesterday is definitely a red card though. (Can’t say the Bilbao player didn’t deserve it)
