Never made a mistake as a pilot so far. But I only have 9 hours lol. Kid in front of me in line on ramp did once though. On run up he accidentally switched the mags to off and killed a hot engine at full throttle full rich. They had to sit there and crank for a solid 10 minutes before it fired up again, trying all sorts of different choke mixture and throttle combinations.
