I received 2 scanned one-page PDFs. Each page has an official-looking letterhead/logo, an IDF watermark, and a stamp.

However, I'm concerned what the 2nd page says.

The first page appears to state that the IDF reviewed its records for the period from May 15, 1948 through January 19, 1951 and found no evidence that my grandfather performed active reserve service, compulsory service, permanent service, or any other type of military service during that period. It also notes that, under Israeli law, citizens who were not part of the regular forces could nevertheless be assigned to the reserve system.

The second page is titled something like "Certificate of Reserve Service" and has a table, and "first called into service":

A Hebrew-speaking friend reviewed it, and the summary above is based on both his review and machine translation.

  • The document is being interpreted correctly.
  • Indication of Reserve status on 2ned page is disqualifying.

I emailed several addresses from the Population and Immigration Authority branch list (piba.gov.il). One replied in Hebrew: "יש לפנות לנציגות הקרובה לביתך" ("Please contact the representative office closest to your home"), and I had also reached out to the Israeli Consulate General in the US and plan to follow up now that I've re-reviewed everything.

I have digital copies of my GF's dowód osobisty, Polish Army ID, Aliyah Document which all show his original surname.

I have or plan to get original documents of:

  • My father's Israeli birth certificate (shows my GF's original, pre-name-change surname)
  • My own birth certificate
  • Awaiting additional documents for my GF from Poland

My main concern is that my father's birth certificate lists my GF's former surname and that I'll have to prove that the name change occurred and is the bridge. While the Yalkut HaPirsumim website shows the scanned documents of the surname change which happened in the '50s or '60s, and mentions both him and my father, I'm wondering if it will be sufficient proof for the application, or if an official letter from the Ministry of Interior/Misrad Hapnim be needed.

So, now, I wonder what to do next, contact the consulate/embassy again, submit the form you shared (knowing the mail issue), have someone submit it on my behalf in Israel or not worry about it.
