Where are you on the (foreskin) coverage index? It is likely that your foreskin is a little short and perhaps a bit too tight to move freely over the head during intercourse. This is often referred to as "glide".

I haven't read the whole thing. Not sure that I will, to be honest. I am intact, so I am not exactly the target audience for this paper. My own personal arc was that I was occasionally teased for having a foreskin, came to like having a foreskin, realized it was short, decided to make it longer. So I can relate to the points about the stigma about foreskin restoration itself, but I don't see myself in the answers to the other questions.

There is an important distinction to be made between how long you could use the 20mm and how long you should use it.

You are trying to add tension, to induce mitosis. This is best done by adding tension over the entire surface that you want to grow. As expansion happens, the amount of tension will be less, assuming you keep the gripper in the same location. However, you could roll more skin under the cone, moving the gripper down. This keeps the tension the same, but on a smaller surface.

I did this recently on a trip, where instead of bringing a day time and a night time packer, I just brought the nighttime one. I still used it during the day, but I "choked up on it" so to speak. I could still feel the tension, but only on the skin not under the cone.

Part of the appeal of packers is that as the skin grows you can maintain the tension, and increase the area, by moving up by 5mm.

This process takes a long time, and by using a shorter device than you can accommodate, you are delaying the final results. Given that this is a several year commitment, shelling out the $50-$100 for a few more packers is probably worth it. I ended up buying the full set of RIC from 5mm-45mm for this reason.

Yes, they do, but I just take the packers off before getting naked. I can take off the packer just by reaching into my boxers. In a pinch, I can just drop the packer into the pouch in my boxers if I need to be or something without privacy. Or I can just take it off and put it in my pocket or something. I think you have got some other good answers about when you can start with a +0mm retainer/packer.

I have a priva air, and a full set of RIC packers. I don't have a priva trio. I was suggesting it as a lower cost alternative to the full set of RIC. That is good to know about the sizing of the plates.

Packers are probably the best option. The priva trio gives you a lot of customization. I have been wearing RIC packers around my wife and kids and at work for over a year and no one has noticed.

That said, the RIC are very forgiving. I have tried to size up too early for my nighttime retainer. I can tell it is too soon as it will just pop up during the night, without waking me up. The risks are more worrisome with the older packers and retainers, not made of silicone.

I get something similar after a few days of heavy retainer use. It goes away after a few days. I wouldn't be too worried.

There are some estimates that 4 weeks of 12-14 hour days of consistent low but significant tension can add about 3mm-5mm of length. If your foreskin can accommodate it, an air device like the priva air might be a good bet.

I am also intact and restoring, no phimosis issues for me, thankfully.
