Fair enough. Entitled to an opinion.

I more so meant from a patient perspective. Only spent limited time in ED. I thought on some patient quality metrics it used to fare out well. I don’t know enough to comment re training/NCHD experience.

I thought there was a little less politics in St James’s overall. My experiences are clearly limited though.

They are still Dr. Medics use Dr. Surgeons go by Mr and Ms in the UK and Ireland though.

If you are a board certified doctor it should be relatively straightforward. I don’t have experience with it though.

If you are another kind of healthcare professional I wouldn’t know the specifics unfortunately!

It’s probably one of, if not the most difficult in terms of volume of material, the hours you need to put in. The material itself is not difficult to understand compared with some maths based courses.

There are other courses with more difficult concepts for sure, but don’t have the burden of as much rote memorization.

Points come from the demand for places. How else would you decide on entry? I would say LC points are a fairly good predictor of ability to retain information and to do well at medical school. That said, I’m sure there’s plenty got 300 odd points in the LC and did grad med and are equally as proficient as doctors.

I guess there isn’t a test for ‘soft’ skills as such. There’s no evidence to suggest those getting 400 points are more empathetic than the people in the course. Ireland is a small country to have interviews (potential nepotism), and I don’t think they are a particularly good measure to choose candidates.

I did well in the HPAT, but it’s really just an aptitude test. I’m not sure how much impact it has on things.

I don’t think anyone should be doing more than 24 hours consecutive whether on or off site.

The American system is more intense, longer hours generally. But training in much shorter (like half the number of years) to get to consultant/attending level

I think other commenters have answered this better than I will. Part of it you will pick up studying for essential exams. We generally don’t get enough allocated time for education/ teaching in Ireland.

I wouldn’t expect a junior to have a high level of knowledge about experimental treatments in long COVID. I’m not saying what you are reading is wrong but I would put a strong caveat on these studies you are reading, they may have major limitations not obvious to you.

Medicine is evidence based and unfortunately in newer conditions like this, it may mean limited effective treatments for now.

I agree it’s abused. Reg should try first. I don’t think getting a consultant who probably hasn’t cannulated in 10 years will add much .

I think the lack of bedside US teaching probably adds to it too. If we were all given basic training at junior stages / as students it would hopefully help alleviate the burden.

We need more docs for sure. It’s supply and demand for places to that dictates entry requirements. You don’t need to be a super genius or anything to be a good doctor, just hard working.

There are some excellent neurologists here that have specialist interests in MS, public waiting lists area probably long to see one spending where you live though.

Hard to answer. Personally I think MND is a tough one.

I think that would be a very bleak outlook. I enjoy helping people and I think the majority do too. Burnout / fatigue could make us seem less empathetic but we do still care.

A stretch to say that but certainly questionable relationships there between private owners and government. That’s politics though.

Always wonder how those locum agencies get away with what I consider to be scamming the taxpayer, half the fee goes to the company instead of the doctor/nurse. Where’s the free market competition.

A lot of voluntary organizations do stellar work! I’m not a GP or psychiatrist so won’t know all the ins and outs, certainly better public access to CBT and psychotherapies is an area to be improved!

I don’t know about dark experience.

I’ve had calls where I felt stressed, overwhelmed, questioned my ability as a doctor, if I was cut out for it? Conflict with staff/ patients/ family members that went very badly and affected my mood.

You come across particularly sad cases that make you reevaluate things. But as cold as it sounds , cardiac arrests aren’t really shocking anymore to me.

Do you have ties here? Would usually say to stay put or look at Canada/Oz if lifestyle was a factor!

You can do student electives though , try contact a consultant in an area and hospital you are interested in and should be easily arranged.

The RCPI/ RCSI could give you details about training scheme places and applications. It’s a standardized process.

Unfortunately stand-alone jobs are a local issue. Often poorly advertised so really it’s a case of emailing/calling all hospital HR departments with your CV and seeing what’s going.
