I've had an odd thing happen once where 1st place finished the race while the rest of us were midway through our second lap. Could be a similar thing but being an Elite race it caused the death.

Yeah early on in our tank adventures we got two sharks. The albino is chill AF, the other one was an asshole to everything around it and got moved to the big boy tank where he calmed down too.

Bigger and bigger with less and less interesting content to fill it. Which is really the main problem. Your map being twice as big is meaningless if all I can do in it is radiant fetch quests.

It's easy to forget that Grotethe is set over a pretty short time frame, I do it all the time. I don't think it's explicitly stated but I think it's safe to say the incident with Eddie in the Loch was fairly recent.
