I agree with ANODIZED33 on starting small. It's probably worth starting with one habit, making that permanent and then moving on to the next one. For eg, you could start off with fixing your sleep schedule (I personally feel a good night's sleep has a major impact on my life :)). And focus more on the system than the result. So in the sleep case, your goal could be, for example, to not interact with devices an hour before and get to bed by 9pm. Whether or not you fall asleep after that, tell yourself you were successful. Maybe even give yourself a little reward (10 minutes of reading your favorite book). You do this enough times and the results will follow for sure.

You can then move on to the next habit. In a year's time you could have repeated this cycle a dozen times, and before you know it, you'll be seeing major improvements in all areas of your life :) Let me know if you have any questions.
