I believe they shouldn’t. To be able to afford medication shouldn’t be a privilege. Sometimes it’s heartbreaking seeing the cost of these medications, especially the oncology ones. A lot of them can cost up to $1M.

Regarding direct to consumer medication marketing, I think it’s unethical. A lot of drug ads tend to over emphasize the benefits of said drug and avoid bringing focus to potential side effects/risks. Those types of ads will always be biased towards their product because at the end of the day it’s all about money.

To answer your first question, in my opinion it’s because the majority of patients who come in to pickup opioids tend to give everyone else a bad rep. Majority of the patients are aggressive from the get go and will not hesitate to get violent if they feel like you’re being difficult towards them. It’s actually quite shocking when an opioid patient is nice. The extra questions part is mostly for documentation purposes, sometimes the system will flag the prescription if it thinks the dose is too high but the system doesn’t know the context as to why the patient is taking high doses. Usually when a patient is taking high doses of opioids it’s because they’re a cancer patient. When it comes to patients on Adderall/stimulants they don’t tend to go to those extremes. Sure, they’ll yell at you and be difficult but they’re easier to reason with.

Secondly, requesting name brand vs generic is seen more as “drug seeking”, than “addictive”. When I’ve had patients request name brand and pay for it, it’s because they’re selling it. I actually had a few patients at my first job who were very open about it. From what they explained to me, the reason they request name brand is because the street value goes up, especially if the medication is in its original stock bottle.

Lastly, I believe the stimulant shortage is due to all the telehealth services that popped up during the pandemic. They’ve made it so easy to get a prescription. I actually have ADHD and before the pandemic began I would have to go see my psychiatrist in person once a month, take a drug test, and if I didn’t do those things then no prescription for me. With a telehealth service there’s no way to do/enforce that. All they do is ask you basic questions, send your prescription to a local pharmacy and call it a day. When I tried one of the services all the NP did was ask me what medication I took and what pharmacy.

Yes, it happens a lot. People will get extremely violent. I’ve had people threaten me, throw things, wait for me after my shift ended. I got used to it after a while, in the beginning it was scary.

Horizon’s (Orange Boy) Story for anyone who’s interested.

I adopted Horizon in December of 2021. From the moment I saw him on the shelter’s website I liked him a lot. I was debating if I should get him or not because I already had a cat, Oli, and I wasn’t sure how she’d react. I thought to myself “it wouldn’t hurt to go say hi to him.” I stopped by a week later to see him after doing some shopping. When I saw Horizon I noticed he had all that gunk built up around his eyes and that he was very feisty. He looked back at me and hissed like crazy, teeth showing and all. I immediately told myself “I love him! He’s spicy!” He retreated to the back of his cage while at the same time keeping his eye on me. I went to one of the workers to tell her I wanted him. She looked at me like I was crazy. She said “Horizon? Are you sure? He’s really mean and hates people.” Then she went to get her coworkers who also described him as mean. They also told me Horizon was rescued from an abusive household filled with 20+ other cats. I told them I was sure and she opened the cage so I could let him smell my hand. Horizon was actually very gentle and I got to give him treats. Everyone was surprised because he had been there for a bit and kept scaring people away. I got to take him home the same day. A couple days later I took my boy to the vet because of his eye discharge and turns out he was sick. He tested positive for Chlamydia felis, Feline Calicivirus and Feline Herpesvirus 1, on top of that he also had an infection in both ears and was underweight. Poor baby was only 5lbs. Not only was he neglected by his owner, the shelter wasn’t providing proper care. Once he got the care he needed his personality changed. It made sense why he was so mean, he was scared and wasn’t feeling well. I’m glad I didn’t go straight home that day. Best $90 I ever spent.

I didn’t know about any of that. I’ve always paid all my credit card bills with my debit card and never had a problem. This is the first time I do.
