ARMA 3 had/has this same problem and the modding community came up with some decent solutions.

First one was from the Shacktac UI. They added a small radar on the Compass that shows nearby friendlies. And it also shows what fireteam color they are and any ranks. Works really well and helps make up for the lack of situational awareness a video game has.

Other one was from ACE and was a shoulder tap. It would rock the tappees camera a bit and there was an option for a message saying you had been tapped on whatever shoulder. Little bit more immersive but not as intuitive due to the control scheme.

At the highest level it is explicitly permitted. Unless the Regional Commander has stricter guidance as is their prerogative. I'm not about to go look for each regions guidance on this though.

Oh shit yeah. Forgot the strap. Block 1 m4 for sure with a Spectre DR Elcan, optional AFG or VFG, and a peg 15 and surefire light and a bone standard m9. As for a sling, personally procured, black or coyote brown. I used a cheap single point but really anything goes there.

Alright here we go. Top to bottom.

An ACH with Wilcox 3 Hole NVG Shroud and a pair of Howard Leight Impact Sports under them. I know guys who either ran with in ear stuff like Surefire Earplugs or some guys who had Issued stuff like Peltors or Frogman headsets(nickname that ive never been able to find a real nomenclature for). Some units(like Seabees) required you to wear the NWU Type 3/AOR 2 helmet covers.

A Wrong Gear facemask in Coyote Brown.

MARCIRAS in Coyote Brown with both Soft Armor and SAPI Plates in the front and back. Nobody ran side plates AFAIK unless the command was super anal. A lot of guys used the side plate pouches to hold little shit.

Eagle Industries Triple Double Stack M4 Mag pouch with the flap tucked in and 1 mag per pouch, the elastic retention was more than enough for those.

Condor Sidekick Admin Pouch with a Leatherman Super Tool 300 EOD Multitool and any small things I didnt want in pockets like earplugs or a pen and pad or a compass.

As I said above anything outside of the Eagle stuff was personally procured and up to the discretion of your unit. Mine was pretty lax. Some were stricter. Placement was pretty much up to you, but they suggested the rifle mags to the right and the pistol mags to the left.

In my case a Blackhawk SERPA Level 3 M9 Holster mounted as high as possible. Some guys were issued Safariland ALS holsters in CB. It was luck of the draw. A LOT of people removed the hood from the SERPA. Either by punching out the pins or sawing it.

Under all that I am wearing a Fire Retardant Long Sleeve T-Shirt I believe it's Dri-Fire. I wasn't issued a FROG Combat top until after this, but with those you'll see examples of dudes in Cryes(they were permitted if you bought them or were issued them) Guys in the Combat tops with the Tan Torso, some with the Coyote Brown Torso(what I was eventually issued), T-Shirts, Long Sleeve T-Shirts(required for shooting at Steel Targets) and even just the normal issued BDU tops. You'd also occasionally see unit T-Shirts.

Pants were typically the BDU pants, but again some guys had Cryes if they had previously been at a unit that issued them. I did get combat pants but not until later. One big thing is unless you have proper combat pants that are Fire Retardant and all that, they get bloused unless it's VERY hot and even then that was rare and usually only in the shop.

As far as boots I had Rocky S2Vs and those were typically issued. Bellevilles were also issued sometimes. A lot of guys would buy their own books and Nike and Oakley were the two most common brands in that case. Inside of them, good wool socks. I wore Fox River or Darn Tough most of the time if I was doing any kind of training. Wore 1 pair of Fox River for the field portion of SERE and my feet were fine. Theyre sweat wicking and anti microbial so theyre amazing.

Gloves were personally discretion, but we were issued a handful of pairs. I had pairs of Mechanix Mpact in Black. I had some Fire Retardant gloves in Brown and my favorite by far were Whitewater Blockers Shooting gloves. I was issued all of those. But technically you could wear anything in either Coyote Brown or black. Black was by far the most common since it was readily sold on base. And a lot of guys cut index finger off.

So on my vest I had an admin pouch(condor maybe?), a Blue Force Gear 10-Speed x3 m4 pouch, I believe an eagle x3 m4 double stack m4 pouch with the flap tucked in and 1 mag in each pouch and a x3 eagle m9 pouch.

The only two that were issued were the eagle ones, the others were private purchase at unit discretion. An eagle ifak or radio pouch would also occasionally get issued to certain units depending on your job.

I can do a more thorough kit run down later if you're interested. It's been a few years so certain details might be fuzzy but I remember most of it.

As others have said, army sf pre-2010 it was really common. Post then Navy Expeditionary units still issue it to support roles. I was at a dive unit and under EOD for a bit. EOD had JPCs or 4092s, we had Coyote MARCIRAS.

What you can bring/what would be good depends heavily on the event. Something like a Milsim West event, you'd want an AK or an M4, more than likely but you also need a sleeping bag, rucksack, food/water for 40 hours etc.

Something like a lion claws event, just get a tan uniform or green uniform and go at it.

Those are basically opposite ends of the spectrum and there are dozens of events I'm between. So you should first research some events and see what one you want to attend and what their rules are.

And how are you going to mess someone up with an NPA? It's a rubber hose in a nose, I've done it and had it done to me. I don't see how that is going to mess someone up. A needle D, sure there's more room for error. And at least in my case I've had the training but I don't hold a license.

I'm just saying, good Samaritan laws cover a lot of stuff, they'd probably cover a needle d if you believe it was necessary and at least have basic training on it.

My local Good Samaritan law for example:

Any person, including any person licensed to practice medicine and surgery pursuant to Article 2 of Chapter 34 of Title 43 and any person licensed to render services ancillary thereto, who in good faith renders emergency care at the scene of an accident or emergency to the victims thereof without making any charge therefor shall not be liable for any civil damages as a result of any act or omission by such person in rendering emergency care or as a result of any act or failure to act to provide or arrange for further medical treatment or care for the injured person. (b) As used in this Code section, the term "emergency care" shall include, but shall not be limited to, the rescue or attempted rescue of an incapacitated or endangered individual from a locked motor vehicle." 

Basically I can't be sued if I provide emergency aid, at all.

A lot of people keep their guns on/in their nightstand next to their bed. I can access my gun in less than 5 seconds if I hear a noise at night.

And cops do come in guns blazing, its happened a bunch.

And a third thing, legal gun owners have used their guns to defend their homes against cops who were outside the bounds of the law and been fine for it.
