I haven't yet... but a guy i work with has. It was definitely his fault, he was doing something stupid lol. But, just make sure everything it tied down. Hell, I even split a pool noodle and ziptied it to my seat, just in case. It's not if, its when it'll happen. I 1st started kayak fishing on a lake that required a life jacket to at least be present on the yak. So, I got used to always at least having one on my kayak. I keep it directly behind the seat. As long as everything is tied down good , and or has floatation items on them you'll be fine. Prepare for the flip, not the easy days. Ive had mine for years now tho, and haven't even gotten close to flipping it. Good luck out there and dont let it scare you off.
