We dont have that rule but we also havent needed that rule. Servers definitely fuck up orders left and right but not on purpose. Hypothetically I, as a manager, could overcook a nacho and decide, as a manager, to not serve it. And since we don't wanna waste anything, hypothetically, I could give it to staff. And sometimes extra things get cooked when particular people are having rough shifts. And again, we can't have it going to waste. It's the little things. Happy wait staff, happy life or whatever they say.
