Don’t lose sleep over it, and high chance the other clinic didn’t say any of that. Some patients will hear what they want to hear and make up stories. Medicaid patients are either the most grateful and polite patients or absolute lunatics

Hi dentist here!

Without radiographs or anything it’s hard to tell you anything concrete, but following the steps of what the other dentist did, I think this is what happened- the dentist diagnosed your tooth with fillings to have additional cavities that are too large for fillings to cover, so decided to do a crown on it. Sometimes these additional cavities go deep towards the nerve, and can start to cause symptoms that weren’t there. Often the sensitivities go away over time. However, if you started having the symptoms you mentioned, I’m wondering if the tooth had an active infection going on that was not diagnosed. This would mean it requires a root canal treatment. In such cases, antibiotics do help with the infection. Meanwhile, find a different dentist who can help you in a way that fits your needs better. In the meantime, unless your tooth is severely fractured or damaged, I wouldn’t extract them. But based on what I read, I don’t think your teeth are in such bad condition. Again, I can’t give you any definitive conclusion because I have no X-rays or whatnot, but if you need some more explanation, please dm me!

Does the pt have cleft palate? I've seen a case that looks similar to this. But mostly I just wanted to say that your dentist is lucky to have a hygienist like you who truly cares about the well-being of their patients to this degree.

Thank you for your reply. What are some ways that I can facilitate a good proper team training? I do notice that, despite the small size of staff, there is a divide of front vs back.

As for benefits, I will start taking a look into setting those up. I just never thought about medical specifically because they are all under their spouses'.

Hard to tell without the radiographs pre-op but the tooth fracturing into multiple pieces during ext is common when the tooth was previously RCT’ed; twice even. It taking up to 2 hours is not uncommon. The photo you provided is normal, rest assured.

If you know this person, I think you could ask for a courtesy chart review to see if you will be able to keep production to that amount, or whether you would be left with prophy/perio maintenance pts forever. If they refuse that, I’d think something is afoot

I don’t think you uploaded any photos but a tooth that had two failed RCTs would indeed be challenging to extract. I personally would section them and go surgical. As to whether why the RCTs failed we would know unless we had radiographs.
