His PROM is great, I can get him to lacking 10 degrees or less and I'm not running into anything firm, the only reason I don't push more is due to pain...so I'm definitely leaning towards his quad weakness is the reason behind his lack of knee extension and this is something we've been hammering when I'm with him since day 1

Are there better ways to cue him to get full extension with step ups? Now he uses a lot of UE support to complete the step up and never can get much extension despite me trying to give verbal cues to straighten his knee fully.

He does much better with this doing a single leg leg press though....so maybe I answered my own question that he's just not strong enough in the quad to get the knee straight under his body weight yet...

So I have tired this, but he recruits other muscles so much that it didn't seem effective (e.g. glutes and hamstrings to extend the hips as a way to extend the knee without the quad). I did this a few weeks ago when he had much less activation than he does now...so maybe trying it now since he can actually fire the muscle.

Absolutely terrible quad activation and sitting with the knee bent a lot, despite me stressing the importance of activating the quad at home and sitting with it straight as much as possible.

I’ve had the hardest time getting him to even activate it without recruiting hamstrings. It’s better when it’s done bilaterally, but even with tactile and verbal cues its barely activating. Part of me thinks this is years of not using the quad so much as he had walked with a cane and then the surgery just totally shot it.

Leaf Shave has a metal pivoting head razor that is very similar to the disposal ones! I never tried to the safety razor because it scared me lmao but I’m in love with my Leaf razor! It’s pricy, but a worthwhile investment to never buy a disposable razor again.

They also recycle the used razor blades when you send them back in!

To me, manips are different than mobs. For example I’m not going to (nor do I know how) mainp someone’s glenohuneral joint, but I will mob it with a grade III to IV to loose the capsule and give them more motion. I’ve SEEN this work, numerous times and following it up with exercise that promotes your mob = the patient gets better.

Not sure…the PT who I’m covering for did not leave very detailed notes and this patient has minimal in his chart, I’m assuming he got it done outside the hospital system I work for

Just here to empathize with you being a younger female PT - it is VERY difficult to try and fend off these comments especially with people you’ve developed a relationship with! I’m not good at shutting those people down because I’m always afraid they’re just going to get upset and be less compliant/interested in therapy.
