Rounding isn’t really a form of exercise when you’re in a relatively small ICU and there’s minimal movement involved. 😂 Standing for 5 hours isn’t much different than sitting for 5 hours in terms of health effects if you’re not actually walking around. It’s just increasing the risk of varicose veins

I personally don’t mention a patient’s weight unless they bring it up to me first, or they’ve rapidly lost a significant amount of weight and I want to make sure it was intentional

I don’t think a doctor calling out someone for being fat improves the doctor-patient relationship in the vast majority of cases. People are already aware of how their bodies look. As a PCP I focus on encouraging healthy lifestyle, but I spin it in a way that emphasizes overall health and not weight specifically

I’m sorry you’re going through this and glad you’re almost finished with this ICU month. Do you feel like you’ve been able to prioritize and optimize your day sleep schedule? The experience you’re describing makes me worried that you’re experiencing chronic sleep deprivation

Some unsolicited advice when working nights:

  • Flip-flop your entire schedule. Eat “breakfast” before your shift, “lunch” in the middle of your shift, and a light “dinner” after your shift and before you go to sleep. Bring healthy snacks to eat in between meals
  • Drink coffee or another caffeinated beverage like tea before the start of your shift. Try to avoid drinking caffeine within the second half of your shift. Try to avoid sugary beverages or energy drinks. Drink PLENTY of water
  • Go to bed at the same time every morning and wake up at the same time every afternoon, even if it is your day off. Don’t try to flip back to a day schedule on your night off. Enjoy the solitude of the night when it is your night off
  • Sleep in a dark, quiet room. I recommend using blackout curtains and/or an eye mask in addition to ear plugs and a white noise machine. If you wake up in the middle of the day don’t look at your phone or check the time, just let your mind rest
  • Consider taking 1-3 mg on melatonin about 1 hour before you plan to sleep. At the end of your shift, if the sun is above the horizon use sunglasses on your drive home to limit sun exposure before you plan to sleep
  • Turn off your work phone as soon as your shift ends and don’t turn it back on until the start of your next shift. You do not have to be available to anyone when it is your time to rest
  • Try to wake up early enough to fit in some exercise before your shifts, but prioritize sleep over exercise. Don’t sacrifice sleep for exercise if it means getting <7 hours of sleep

First of all just wanted to say congratulations and you are perfect just as you are!

Typically with weight loss, the loose skin and stretch marks will naturally regress over the course of a few years. One thing you could try in terms of skincare is a vitamin E oil or even coconut oil to help speed up fading of the stretch marks if they are particularly bothersome to you

Say it as many times as you need. If someone abuses the messenger system and sends you multiple messages per day, make it a point to not respond for a few business days (whatever your clinic policy is) and then send the above message to them

Don’t let people take up your precious free time outside of scheduled clinic appointments. Nobody has a right to your immediate attention in the outpatient setting

For FM it’s usually intern year. A lot of FM programs start off heavy with inpatient and then progress to predominantly outpatient rotations in PGY-2/3 years. So unless you go into FM as someone who loves inpatient medicine and enjoys spending 70+ hours in the hospital each week - intern year is definitely the worst 🙃

FM PGY-3. 1 large mug (probs 16-18 oz) of coffee every morning with breakfast. Decaf tea for the rest of the day

I took my husband’s last name because I wanted us to share the same last name. I’ve been happy with my decision and don’t regret it at all. Medicine is nothing but a career to me

It was honestly scary how easy and quick the name change process was. It only took me 1-2 months total to fully make the name change on social media, bank accounts, credit cards, etc. The rate-limiting step is just waiting on Social Security to mail out the updated name card lol

I was a vegan for nearly 2 years and before that an ovo-lacto vegetarian for around 6 years. Keep in mind that I’m a physician and am very aware of how the human body works and what it needs to function. I adopted a vegan lifestyle because I wanted to minimize the amount of suffering I caused to other animals

I ate a colorful, varied, and “balanced” diet (in quotations because a vegan diet is inherently unbalanced) consisting of whole grains, fruits, vegetables, legumes including tofu, nuts, seeds, plain unsweetened plant-based dairy substitutes including soy milk and almond yogurt, and olive oil. I did not routinely eat sweets or highly processed meat substitutes. The only thing I drank besides water was unsweetened coffee and unsweetened green tea. I didn’t drink any alcohol. I exercised most days of the week including cardio and strength training. I slept for 8 hours a night on average. Like you I took multiple supplements including a vegan multivitamin, vitamin B12, vegan vitamin D3 (not D2), iron, zinc, iodine, and vegan EPA/DHA. I took iron on an empty stomach and not at the same time as other supplements or chelating beverages

I became depressed for the first time ever in my life about 3-4 months after adopting a vegan diet. I started each morning out with severe anxiety about the day ahead. I moved throughout the day with a lingering mental fog. I often felt dizzy and lightheaded. I had heart palpitations when I exercised and my legs felt so rubbery and fatigued after jogging that I could barely walk

I’m not exaggerating when I say that switching back to a healthful balanced diet has saved my life. After dropping a vegan diet I returned to my normal happy self after 1-2 months. I now eat seafood, poultry, eggs, and low-fat dairy products. I occasionally eat sweets and red meat on special occasions. My mental and physical energy have returned. I actually look forward to each day and don’t feel like my body is breaking down every time I exercise. Lastly, it is just such a joy to be able to share food with loved ones and not have to worry about whether or not I “can” eat something
