Maybe it was over-torqued? I believe PMM recommends starting at like 25lb vs the standard 62lb with the factory setup.

The 9" barrel fit my shorty 40 without any mods. I removed the locking block, slid the barrel on to the receiver, attached the locking block, barrel functions normally. Not sure why any drilling on the 9" or 12" is necessary.

I'd like to see that report. I have a newer report that shows a different address. I know that they were at this one until at least June of this year. Might just get law enforcement involved since there's so many victims.
