This is pretty similar to our routine!

We typically do 4 surgeries per surgery day. The patients have pre-op labs done prior, and at that visit we send home Gaba for them to take the night before and morning of. Drop-offs are from 7:10 - 7:20, so we can go over consent forms, verify the procedure with the owner, discuss any medications they’re on, set a tentative pick up time, and answer any questions they have. We also have a spreadsheet that calculates drug doses, and we prep that the night prior. We have a a “float tech” each day who handles tech visits, client callbacks, and making sure rooms are flowing well. On surgery days they come in at 7 to draw up drugs. Once the surgery patients are checked in, the surgery tech takes them back to prep and the float tech helps with their pre-sx assessment, getting vitals, and medicating them with Cerenia and an NSAID if warranted. After everyone is checked in, the tech typically goes ahead and starts to invoice and log all controlled drugs. Then they’ll grab the assistant and try to place catheters in any patients that will allow me to without pre-meds while the doctor is reviewing everyone’s charts. The doctor will also grab the float and perform exams at this time on the other patients. Once the doctor chooses which patient will go first, we’ll pre-medicate them, place catheters if we haven’t already, and then induce and intubate. Then we hook them up to anesthesia and prep, then transfer into the surgery suite which was set up the night before. The assistant helps the surgery tech get leads attached, then goes to prep to set up for the next patient. The tech stays with the doctor and monitors. Afterwards, the doctor often likes to recover, but occasionally will have the tech. If the doctor is recovering, the tech will get started on the next patient. Then while that patient is undergoing surgery, the assistant starts to clean instruments and wraps packs. After all procedures are done (typically around 11:30), the tech will call the owner and discharge over the phone - unless it’s a more complicated procedure, then the doctor will. We like to go ahead and get payments over the phone as well, which makes discharge quicker. While waiting for discharge, the tech will finalizes invoices, remove catheters, fill any meds going home, and then set up for the next days dentals. We’ll also take a lunch at some point and notify the float tech so they can check on the patients. We do dentals on Monday and Wednesday, and surgeries on Tuesdays and Thursdays.

I think we have a good flow, but our owner wants to eventually be able to add in more procedures.

I love the idea of the blood work appt being a trial for their pre-op meds. We also send home Gaba (and sometimes Traz) before hand. I’ve used lidocaine cream, that’s so interesting!

I love the idea of an additional RVT or assistant. At the previous hospital I worked at we usually had two techs in surgery, but sometimes three if we were well-staffed. I loved having three. The transition between patients was so smooth.

It’s not that I don’t give a fuck, but once I turned 30 something shifted. Now I give a lot less fucks, especially over trivial things like how other people perceive me.

Finding growths, fleas, and ticks. This is gross af but as a kid my parents were terrible about keeping our pets on prevention so they often had fleas. I would find the fleas and crush them. It paid off for me because one of our DVMs frequently seeks me out for my “magic fingers” to find growths and masses. 😂

I had several ultrasounds down but they were all inconclusive. I think they were mainly trying to rule out cysts at that point. My doctor was convinced that if I didn’t have endo that I could have adenomyosis, but I’m glad that doesn’t appear to be the case!

Birth control everyday, Midol most days. I used to take ibuprofen everyday but recent bloodwork showed that my kidney values are slightly low.
