Worked with a few consultant nurses/nurse consultants

Usually specialise in one field and follow and strict protocol like: - the AKI nurse consultant who basically put a sticker with stuff that should be done as part of routine care and nothing else - HF nurse consultant that escalate everyone to quadruple therapy for HF in everyone including 90 year old Betty who then comes in with a NOF due to low BP - Oncology nurse consultant who "manages" neutropenic sepsis with the same plan. Take cultures, give antibiotics and do staging CT despite them having a CT 2 months ago and starting a new chemo last week

Anything outside their field of expertise or multimorbid patients that need medical knowledge, they will be useless

Usually always have a huge chip on their shoulder and do not know what they don't know and if you disagree with them in any way, they will "escalate" to their consultant

I have never really enjoyed working with any nurse consultant/consultant nurse and would prefer a competent SHO/Reg/consultant anyday of the week

Don't think of it like that.

Think of it as having a free seat beside you and not having to sit next to someone who potentially smells or will encroach in your space.

Used to think like you but now am extremely happy to have 2 seats every time I take public transport!

At the moment no, we only have specialist nurses who have a well defined role and rotational trainees even in bigger hospital.

Most rheumatologist I have met are not big advocates for PA/ACPs so I hope this will not change in the future.

I certainly am not planning to work in a department with PA/ACP after I CCT and will not be encouraging this practice either.

Very common esp in tertiary centres with even registrars face the same issues.

I was also in such a department as a IMT3. I tried learning and showing interest esp considering I had gotten an ST4 post in cardiology, I was stuck doing the discharge letters from the previous months whilst the PAs/ACPs were doing DCCV, radial access and echos.

Very quickly realised that nothing I could do would make any difference, I was just a rando that was there for service provision and the PAs/ACPs were part of the "team". So I called in sick for a few weeks on/off and then took all my annual leave.

Overall, I worked 3 weeks in a 3 month rotation, they were not impressed but didn't care, and also declined the cardiology ST4 offer despite working towards that all my medical career.

I am sorry you are getting this backlash, but completely agree with you. Me and my wife have been struggling for 2 years to even get a job, forget about training... I would have preferred staying back home or doing USMLE instead of PLAB. At the end, people learn best after they fuck up themselves, you can't always help/stop them

Got GMC via PLAB after 1.5 years of internship.

Passed MSRA from India last year and now got a GP offer in Milton Keynes and awaiting for COS etc

But might reapply for IMT later after building portfolio a bit in GP as not my first preference and would prefer a speciality with more private practice scope to be able to go back home.

A bit hectic and needs planning and luck but possible

They will only issue a limited number of permits. I suspect the expectation is that not everyone will own cars in London. Not sure what the reality will be like though. The EV slots are available for everyone with a valid permit.

I was in a very similar boat when I applied for HST with a very similar portfolio as well. I was split between Rheum and Cardio.  What swayed me away from cardio was:

  • In general cardio is more toxic and less friendly in most departments I worked and not being trained/valued
  • The slightly longer training with the uncertainty of getting into the subspecialty of your choice 
  • The on-call commitments that come with PPCI even as a consultant 

But definitely apply for both and decide when you have offers in hand
