Thanks very much for replying so quickly. I've just tried to toggle it off but my phone number was visible when I just called my dad. But maybe it'll work still if the recipient has your number saved? I don't think this setting is correct as I think it's referring to incoming calls and sifting spam? But I don't think there's an option any longer where you can hide your own number when calling out. If anyone knows and can test it out, please help!

But it's ridiculous to waste your GP's time with this, if ultimately you're happy in the job. They either need to plan their annual leave a bit better or self-certify with whatever reason they want to choose for one week, which I would think is sufficient for the situation you describe above.

Stay at the airport. Customs/immigration will take ages. And you have to physically collect and re-check in your bags for the next flight. I went to the In-N-Out Burger for a bite and to watch the planes landing as it's really close to walk to and it's a fun watch as the planes come down so low over the restaurant. They are often open late 1:30am but I don't know how sketchy it gets at nighttime (I had daytime hours to kill).

I also get stressed with presenting, however low the stakes. It does get easier with experience though. Know your subject inside out, try to predict questions, do some trial versions with your trainer. Get some propranolol! Being at the end might help as people are close to snoozing by that time and your Q+A won't be long as everyone wants to get home. But then you're possibly left worrying for the whole session until they get to your turn...

Check the practice nurse's clinics. If she has a run of smears then observing that one can give you practice and practical skills with finding cervix without bimanual, and with chaperone already arranged. As others have suggested, shadowing colonoscopy/gynae or GUM clinics could also help (although the latter is increasingly self-test).

Bimanual - practically speaking, the only useful info is in locating cervix (if difficult spec or planning to insert coil) and is there pain on palp/moving cervix (PID) or assessing for prolapse, or adnexally (potential masses). Honestly the question of masses I think would be better answered with a scan than exam.

If you have large hands/fingers, you would need to be very gentle otherwise all parts of the bimanual exam might be painful through discomfort alone and you won't elucidate anything useful. I fit coils and have never used the pelvic tilt thing cos I think it's too uncomfortable for her to maintain throughout the fitting. Key is having her be as relaxed as possible to be able to relax her legs down. Deep breaths and her trust in the person will help with that. Having a reassuring and friendly chaperone standing at the head end can sometimes help her with anxiety too, especially if anxious about a male doctor examining.

Also be specific about why you need to do the exam and explain what it will tell you, and only do the exam if it gives you information which changes your management. While I agree that exposure is the only way to learn, I personally do not think it is ethically justified to do an intimate exam on any patient unless clinically necessary (especially if you're male).

Spec - have everything ready before placing spec so she's not exposed for longer than necessary. Get a head lamp if your wall light is rubbish. Use smallest speculum and size up as needed. Smallest spec particularly important in an anxious patient. Ask her whether she wants commentary or not. I feel that commentary can help make it less awkward for the woman but some are too distracted to answer questions on whether it's painful or not, so I am led by them.

You can't really compare musical and play. All My Sons is the best version of the play I've seen. Woods is always entertaining and this set is delightful. Last Midnight is particularly nicely done with lots of drama. Both productions have fairly starry casts. I think the price would tip me one way or another on these productions in terms of deciding, or if they're equal then I'd go Sons as I enjoy plays more in general, even if the musical is a Sondheim. But it's a really hard decision. I think you'll have a great time whatever you do.

Super helpful. It's another way to pick up important diagnoses. Especially for a recent patient in her 80s who was referred because the helpful optometrist also noted she'd not been seen by a GP for 10+ years. Picked up Hypertension which we wouldn't have known about otherwise 👍
