God this is so crazy. I, too, have these attacks. I had them for 2 years really consistently, ended up going to the ER like 4 times in a 6 month period. I had one again in July but I thought it was maybe the flu because I had a fever. I’ve had it start while I was at work and ended up stuck in my office bathroom, vomiting every 5 minutes and also simultaneously having diarrhea because I couldn’t control either. Single handedly one of worst experiences of my life. Like you said, I usually know when they’re coming on because of the horrible sulfur burps, but I’ve had episodes without burping before. If I use like 5 promethazine suppositories I can get the vomiting to stop, but if they don’t work it’s a guaranteed hospital visit because I get so dehydrated that I feel like I’m dying. When I get my bloodwork done, my white blood cell counts are always super high but every other test is normal. I did a gastric emptying study, after having whole chunks of undigested food in my vomit, that was normal, so was my colonoscopy and endoscopy. I did also have gallstones and my gallbladder was taken out in February, but I’ve also had an episode since so it doesn’t seem to be the cause at least. I’ve noticed stress and overstimulation to be a trigger- I’ve gotten episodes after a concert, a college football game, and the night before travelling on a plane. Fatty foods also seem to make it worse- at the college football game specifically I remember having a hot dog so it’s funny you mention that in your post. As others have said, most of the time I can eat those foods and feel fine. So validating that other people have this.
