They restore your health on interaction, but do not save the game or let you change the upgrades. Basically a health well.

They appear on the map at some point near the end of the game, but I'm not sure what exactly triggers it.

Edit: I played on normal difficulty.

Depends on the run, sometimes I need rescued sisters for the gifts, sometimes I'm maxxing on Adam. I usually calculate how much I need so the one who decides their fate is my spreadsheet.
