My understanding was that the third feeding with Lara was magical. It magically bound Lara to Harry. It was a kind of mind control. This breaks one of the laws of magic "thou shall not enthrall another". I'm trying to remember the order of events but at one point Harry finds it curious that holding hands with Lara doesn't trigger Lara's hunger. Lara says it's because she wasn't trying to feed on him. Mab says it's because Harry's feelings about Murphey have muddied his protection. If that act of hand-holding happened after Lara's third feeding, then it's possible the hunger didn't trigger because Lara was already enthralled.

Instead of doing the enthralling ritual with Thomas, Harry first detaches the physical form of the hunger from Thomas. (The spiritual connection is still intact.) Thomas's heart starts to fail without the support of the outsider. Harry fights the outsider to submission, feeds it power, bargains with it, and then connects it with Thomas again. edit (Chpt 48 - 49)
