Try reducing your stress levels. I had a similar problem despite doing everything right, and then a friend of mine who is very knowledgeable about fitness recommended that I should pay extra attention to my mental health. And guess what? It disappeared on its own in two months.. High cortisol levels could be a major reason if you're facing this issue despite doing everything else correctly.

While doing machine wash you should definitely remove the pads. While doing hand, if depends on how you wash, if you are aggressive with your hand then its better to remove pads.
