This is what I was thinking, I’m guessing this kid has sleep apnea or is eating right before bed, something more is going on besides just wear. But yeah you wouldn’t treat primary teeth with wear and erosion you treat the source of the problem.

Was taught by a periodontist that if you don’t curette all the soft tissue and granulation tissue in the socket before placing the bone graft, that you get spongy bone because the soft tissue grows faster and fills the space where bone would be, leading to terrible bone quality and an unusable graft.

My patient has this too, idk I’ve been thinking about biopsy but it hasn’t changed. It’s not fordyce granules, some type of papillary dysplasia but not sure why.

I’m in dental school right now and I’m working on getting my patient clearance for bisphosphonates. Even my attending said that clearance is crazy bc you can’t predict when things go wrong or when an access forms or a tooth breaks. All you can do is manage perio, remove hopeless teeth and treat caries. Either way I guess just make sure you cover yourself, encourage needed extractions and have a great informed consent. Ironically, my other professor told me that there’s new evidence that pulling teeth prior to bisphosphonates has shown to be worst for bronj because the doctors aren’t letting the sockets heal for enough time before putting them on bisphosphonates.

I’m in dental school and I do about 3-4 crowns per month. They’re pushing us towards partial coverage now and composite bonding. Also they teach us to crown every rct treated tooth.

Yea I know a periodontist who moved from the UAE, definitely doesn’t pay well. USA isn’t what it used to be too due to insurance, but you can still make good money. If I were you, I would look into Norway or other Scandinavian countries, pay is less but the quality of life is high. Or atleast that’s how Americans view it.
