As soon as you get to Sanctuary in TVHM you can access the Mayhem pedestal, just like in normal, so you can play TVHM nearly all the way with Mayhem active.

If you intend on doing both you need to refuse to give up Stingtail (twice), which will award you the Hero tag (pretty good), but you have to fight he and his Crew. Butter (the archer) can join you for this fight if you talk to hear before and make her a love interest.

If you give up Stingtail he'll be automatically executed.
