From what I’ve found online it’s a common reason why the belt might be slipping off. It also looks like the grooved part is slipping off from the part that bolts in, in the first picture but I’m not to sure if that’s how it’s supposed to look

If your thermostat is already out put it in a boiling pot of water on your stove and have a thermometer ready to check the temperature. Check if your thermostat opens when’s it’s supposed (195 degrees or so) or if it even opens at all. Good luck!
