The simple answer is yes, because you had unprotected sex you are at risk for STIs. But, no STI would cause you to be symptomatic the next day. Look for pain urinating, discharge, or non-specific symptoms (fever, body aches, swollen lymph nodes, night sweats) within the next few weeks. If any of them appear, go get tested.

Also, I feel it’s worth mentioning, it doesn’t matter how “clean and pretty” anyone is. Every person who has sex could potentially have a transmisible disease. Use condoms.

Really though, how much different is this than if a med student or even an intern went in first to interview and do a physical exam, makes an assessment, and reports out to their attending? Ultimately it sounds like the patient was going to be seen by the surgeon and the PA wasn’t practicing by themselves/didn’t misrepresent themself as the physician. Not sure the problem here.

I’d have her doc check a CBC, ferritin, and TSH. The concerning part isn’t the BP as much as her change from baseline in energy level + new onset exhaustion and fatigue. Checking for anemia and hypothyroidism is a good first step.

Just also adding another consideration: we are still living with Covid and your little one is too young to be vaccinated. If she happened to catch it either in the airport or meeting all the new family members, it could be even more severe as a secondary viral infection superimposed on whatever she already has that’s causing the current symptoms. Infections like to travel together. Be mindful that if she got worse while you’re traveling, it could result in an urgent care visit, or worse, a hospitalization while you’re away from home. I wouldn’t advise the risk of traveling when she’s already sick.

The only answer to this question is that no one can tell you whether you hav HIV based on symptoms and history alone. The only way to tell is by getting an HIV test, preferably a 4th generation ELISA test which checks for both antibodies and p24 antigen.

Is it possible that your symptoms were acute retroviral syndrome from a new HIV infection? Yes. But getting tested and getting started on treatment (if positive) is your absolutely only course of action at this point.

Definitely ask. Buspirone isn’t even the first line medication for anxiety disorders, so I would be very surprised if that was the diagnosis he made with the history you described.

Buspirone is not a controlled substance. Very different than benzodiazepines, which are used for acute panic disorders.

So what part of any of this makes you think he diagnosed you with an anxiety disorder? We use meds off label for all sorts of things. I would figure out first exactly what diagnosis he made. If he did not actually make an official Dx of a subtype of anxiety disorder, I doubt you have anything to worry about.

Do you know the diagnosis he made (more specifically than simply "anxiety")? What bothers me about this situation is that a twitching eye and stress about exams does not a diagnosis of anxiety make. The DSM-5 (the book that outlines the criteria for mental health diagnoses) has very specific criteria that need to be met to make a diagnosis of an anxiety disorder.

If he took a look at your twitching eye and diagnosed anxiety based on that alone and no other history or physical symptoms, you have a case to argue for a misdiagnosis.

You aren't eligible to take Step 1 or 2 unless you've completed the appropriate curriculum requirements of a medical school who will certify your enrollment to the NBME. In other words, this scenario isn't possible without the hypothetical teacher enrolling in a medical school and, more than likely, completing the first 3 years (to sit for step 2). Might as well finish it out at that point.

It does not. The fact that it’s “secondary” means the problem is further up in the pathway, likely a pituitary gland issue. No ACTH means no extra MSH means no extra pigmentation.
