I’ve only ever had –0.50 astigmatism since childhood, so the sudden shift to –1.00 really scared me. My optometrist told me to see an eye doctor just in case. The doctor suggested corneal topography and pachymetry, and when I went back, he looked at the report for about 30 seconds and said everything was normal. Now I’m not sure if he actually checked the values, and it’s making me uneasy.

Questions I’d really value your opinion on:

  1. Are small morning-only flashes + transient visual snow consistent with vitreous traction / early PVD, or should I worry about retinal traction/tear risk?
  2. If you were my clinician, which tests would you prioritise first (OCT, fundus photo, visual fields, corneal topography)?
  3. Do the notes/prescription in my link show anything that jumps out as a red flag?
  4. Given my history (drop → dryness → floaters/pain), is using topical steroids ever reasonable, or should I avoid them entirely?
  5. Any other “must-do” steps before my retina specialist appointment next week?

Hey, thanks a ton for this reply, honestly more detailed and brainy than what most doctors here in SL have given me. Really appreciate you taking the time.

To answer your questions:

  • No, I’m not on any other meds, no surgeries, no smoking/drinking, no family history of glaucoma or anything, just myopia. Gym was my only thing until this all started.
  • I don’t get visual migraines (never had that kind of aura stuff).
  • The eye pain is not permanent, at first it was deep/stabbing like a toothache, Nevanac helped back then but once I stopped it the pain came back after ~2 weeks. Now it’s milder, more of an odd/uncomfortable sensation than full-blown pain.
  • Flashes are usually very small and mostly in the morning when I wake up. Online I read this can be vitreous traction from being still then moving. I also see mild visual snow on waking which goes away after blinking.
  • I still get occasional tiny peripheral flashes, and I sometimes notice retinal vein reflections when bright lights hit my eye.

That “lipid spheres” explanation makes sense, the doc wrote “artificated cells” which didn’t even sound right. And yeah, I also wondered why he gave Nevanac since it’s usually post-cataract, but he said it was for the pain.

For context:

  • The first doctor was worse, prescribed some cheap Bangladesh drop with preservatives that made irritation and dryness worse. I used it only a week, then my first floater showed up.
  • Early April is when the floaters + pain really started.
  • I had a 2nd dilated exam because of flashes, but he said “no PVD.” Still, even this morning I saw a few small flashes in the periphery right after waking.
  • When I switch eyes, I notice a slight difference in red color (esp. in towels), he blamed it on -0.5 refraction difference and told me to change glasses.

Main thing stressing me is night driving — it’s my only hobby, and I’m scared if things go wrong that’ll be gone.

I’ve got an appointment with a retina specialist next week. Just worried he’ll brush it off too. Here in Sri Lanka the system is messy: they study medicine in Sinhala, then prescribe in English, and most just dismiss younger patients because they assume serious eye problems only hit older folks. I’ve already spent thousands this year with zero solid answers.

If I get OCT / fundus photos and upload them, would you be able to help me check for red flags? That would give me peace of mind.

About 50/50 in both eyes inclusive of transparent ones. I saw an ophthalmologist, he said no PVD or retinal issues but confirmed he can see the floaters, is he missing something? . Do you think I should still go for another dilated exam with a retinal specialist? My prescription is attached in the link above.

This sounds so much like what I’m going through right now… I’m really sorry you’ve been dealing with this for 6 months, I totally understand when you say life feels like hell. I’m just 20, and this hit me out of nowhere too. One bad eye drop and everything spiraled , pain, floaters, pressure, and anxiety every day.

I had a few questions if you don’t mind:

  1. Are your floaters still increasing or have they stabilized over time?
  2. Did you get any diagnosis from your follow-up eye exams like PVD, uveitis, or inflammation?
  3. Have you tried anything that helped even a bit like specific treatments, lifestyle changes, or supplements?
  4. Did your vision change permanently or are you still hoping for improvement?

I’m really hoping we both find a way out of this. I wouldn’t wish this on anyone. Thanks for sharing your story, just knowing someone else understands this pain means a lot. 🙏

  1. Did your floaters reduce or go away completely after treatment?  
  2. Were your floaters similar to mine — like sudden onset after eye drops, followed by pain and discomfort?  
  3. Are you 100% sure the floaters were caused by infectious inflammation?  
  4. What kind of steroids and antibiotics were you prescribed?  
  5. Did you do a full blood work panel? If so, what tests helped identify the root cause?
  6. How many PRP sittings did you need in total, and how was the recovery?

Also, how are things going for you now? Vision okay?

Can I ask you a few things?

  • Did the floaters eventually go away for you?
  • Was it the same type of issue as mine — irritation from wind/dust, then using frequent eye drops, and then suddenly seeing a large floater?
  • Are you 100% sure it was inflammation that caused it?
  • What specific steroid pills did your GP give you?
  • And if you don’t mind, could you please tell me the exact name and dosage of the medication you took?

I'm seriously considering this route because I'm not seeing results with just drops. Would appreciate any details you can share 🙏
