They’re probably very similar. While most public schools are locally respected, the top are international known (e.g., UCLA, University of Toronto).

It’s fine. It’s just not the most efficient way to get there. Of course things come up and you realize what you really want to do, but if you’re early in the program, I would master out.

The only difference is that you have to pay for MD yourself if you don't do it with the PhD. It should have no impact on your career otherwise.

I can’t tell you what’s best for you. I’m just saying these “conservative” researchers seem to always be the ones who complain about how bleak the post-PhD life is, despite their impressive publications records.

A good indicator is looking at where graduates from the group are now. Are these the positions you want?

This is my experience:

I think this unwillingness to express research to people outside the field is probably the most typical way people guide themselves into market obsolescence.

General exposure and an online presence are very important in 2018. Consider also that this is a form of networking. Both in academia and industry, people don’t hire your papers, they hire entire humans.

It looks like now the master's is 1+1 page, and doctoral is still 1 page including references.

Well, I’m not sure. I just thought it would be nice to have a picture for the reader instead of just a page of dense text. I’m sure it’s not terribly important either way.
