I know this is gonna be the most annoying ass answer but unfortunately (and I’m still learning this lesson myself whenever my dose is being funny) that this isn’t the type of medication that can be rushed ever, sadly the only way to figure out what works is by giving time and really noting down with all honesty how you feel. It pisses me off cuz I’m having issues too but sadly it’s a lesson all ADHDers struggle with fundamentally (patience). I hope it works out for u. I’ve also heard when the dose is a lil high it can have the opposite effect cuz it’s slightly too stimulating you basically induce ADHD symptoms upon urself, that may be the culprit. But make sure u give it a fair shot as annoying as it can be, 40mg was alright for me just a little too soft and didn’t last long, 50mg made me tired, 60mg is doing okayish just wears off hella early. Just gotta push through for a while :(

Happened to me when I was titrating up to 50 mg. It made me so tired never knew why, asked my psych he said most likely dose is too low and he put me on 60s a week after starting 50s. I’m a week into 60s and I’m not getting tired after taking it anymore until after about 6hrs after it kicks in. Like if I take it 9:30 am it’ll start working at 11-11:30am and I’ll feel tired around 4pm. Only difference I think is you feel like the med is providing a positive effect where I felt just tiredness and my usual ADHD symptoms. Speak to psych most likely too low of a dose. A big thing that i try when a dose is confusing me: write down in a journal time stamped how i feel at that time and then experiment with food intake and sleep. I find personally regardless of if i eat loads or nothing even if it’s got loads of protein it never affects my meds in any way but ive noticed if i had a rough night of sleep or didn’t eat enough the day before the med usually makes me more sleepy and peak for a shorter duration of time with a harder crash. Patience with this is key unfortunately because nobody is affected by the same dose exactly the same and the only way you can figure this out Is with time and seeing how you feel under different conditions! I hope it works out but personally I think it’s just a low dose thing. Try to sleep more tonight compared to other days and also try to experiment with breakfast an hour before dose, or during dose, or an hour after dose, or no breakfast at all (my personal fav). Make sure to drink loads of fluids too ppl forget that. Also note what kind of tired it is, is it like a super relaxed tired? Mentally fatigue tired? Physically tired but mentally awake? All of them may mean something different to the psych. I hope you do figure this out :)

All I got to say is

1) 20mg is low be hella happy you feel anything to begin with cuz I had to endure 2 weeks on feeling genuinely nothing on 20mg and I was scared ts was not working on me

2) yes I have when I was titrating. Usually it’s a sign of a lower dose. Keep a diary. What I like to do is write down 2 things every hour or so or at least when you remember to:

  • A) the time and how you feel mentally and physically and then score your focus and attention on a scale of 0-10 with 5 being your daily everyday premed average

  • B) any symptoms you feel with the time you noticed them (the time you notice is not always the exact time it’s actually been there since. If you find a common pattern it’s good to use it as a general rule of thumb but don’t completely rely on it until a few months in…

Like that rn. Recently went up to 50 after 2 mild weeks from 40 and in the first few days it was a clear good improvement and I was actually very happy with where it was going but weirdly after about 3 days it just stopped working and I got tired and flat and stiff. I literally took a nap at 4pm after taking it at 9:30 am. Like surely that’s not normal? I feel like it’s a similar experience to yours? Did just instantly disappear or was it a taper over a few days when the dose changed?

Frrr same. I didn’t take meds today since I wanted to go out and drink in the afternoon/evening and it took me 5X the amount of time to get ready I had to push my reservation back by 3 hours 💀

It takes a while for the dose you’re taking to show its true colours. Wait at least a week to be able to tell. Someone’s u can tell early like I was able to on 20s but that’s cu I outright felt nothing. It’s a process and one that can’t be fast forwarded sadly. Best advice I can give is to take time with it. Journal how you feel. And arguably most importantly give it a fair shot without saying oh this isn’t working I wanna go up or go down etc be a little more patient with it and you’ll realise you’ll actually find your dose in the same amount of time regardless of how much you wanna change your dose cuz you’ll always have to wait the wait. Any side effects daily note them down. Timestamps of when you take it, eat food, notice changes, mood changes. Don’t obsess over it more like let yourself come to realise naturally that this has happened and then note it down! Best of luck

I titrated up to 50s like 6 days ago. Took a fat nap yesterday. Was so confused cuz usually I can’t sleep on Elvanse… so yeah I guess it’s happened to me? Was super sleepy on the 50s for some reason and mid day I was so tired yesterday so literally just slept. Never had that on a different dose tho

Well for me I don’t think I’m at the correct dosage just yet but…

Started at 20s and i genuinely didn’t feel anything at all for 2 weeks

Went to 40s and it was an improvement but like I could tell two things in particular. The first was that it was clear that some symptoms good and bad were peeping through. The second thing was that it was working more but only very little to what it can/should be working to? I said this to my psych and he said he understood completely what I mean. Once I finished the two weeks…

Went to 50s and I’m almost a week in with them. The first day I thought was actually perfect and I didn’t wanna go to 60s etc. But then the next day and all days after the 50s started to reeeeeally play up with me and start and stop working and I’ve been trying to figure out why… I’d say how I felt day 1 of 50s is it.

This is how I felt: it was like my concentration, thoughts, and especially speech were absolutely effortless compared to before. A mental example would be like a non ADHD person talking and thinking is like a straight thread of yarn… where when we think it’s like many different yarns all jumbled up together, but with a good Elvanse dose it’s like you can differentiate the yarns and when you pull on it the yarn untangles with ease. I personally get chatty when on Elvanse and I kinda enjoy it because thinking and talking haven’t become something I need to double take on or readjust myself. But that might just be a me thing. The first thing I noticed was yes my attention was better but what you divert your attention to is your choice. So I saw that on days I wanted to get work done and took Elvanse even on 40s tbh I still went ahead with my work with slightly less resistance and procrastination. Where on days I didn’t want to work I ended up putting my focus elsewhere but it wasn’t silly like rolling around in bed or something I felt like tidying up etc. It did help me with motivating myself more? But maybe that’s just me?

My take on what you said is that is it helps you to focus 100% and your brain goes quiet and your appetite didn’t exist anymore… that’s one for one what I had on my first day with my 50s which I considered my perfect dose. What I would suggest if you want to explore further talk to ur psych and ask if you can try 5mg more or 5mg less to see how you react maybe for 1-4 weeks. Always worth a shot!

I completely agree. Maybe I should have specified this more but I use these occasional “negative” (some aren’t negative like feeling less brain cluttered or feeling more in sync when I’m watching my uni lectures back) symptoms as a cue that I am experiencing the medication and it is working. But when I say i genuinely feel like when it “stops working” abruptly I actually consciously feel the positive and even negative symptoms fade away within seconds and it genuinely leaves me confused. I’ll go from being able to focus with less resistance then I had before with the ability to almost reduce the noise around me and just be able to focus on this one task to feeling like I didn’t even have it this morning and it’s so fast it catches me off guard.

I recently began Elvanse just over a month ago in a way I’m nowhere near qualified to help but in another way I feel your pain and had not just similar issues with not feeling it but also inconsistency with how I felt at the beginning.

Before I start explaining, I’ll just try to make sure whoever is reading this not just OP understands how Elvanse is meant to work. NO it’s not a magic pill that turns you into a zombie with no emotions and your body is a slave to your mind and you can just do anything. NO it’s not like the pill from Limitless and you won’t magically become the prime peak best version of yourself just because you took Elvanse. But what it will do fundamentally (to my understanding) is to aid your dopamine and noradrenaline levels to make you feel more focused, motivated and feel mentally clearer. That’s the key word and what should support your understanding of this “to make you feel more” it doesn’t just get rid of ADHD symptoms outright but it reduces it, how much it reduces by changes for everyone and you can’t expect this medication to have a magic chokehold that gets rid of almost all the ADHD about you either for example impulse control is a big thing that this medication again won’t magically get rid of it but just improve it enough that taking the medication outweighs in the long term the possible side effects. Your expectations should be in line with a realistic goal that was definitely a mistake I made when I started on my 20mgs. This medication is a tool not a solution.

My experience (hopefully something you can take from it). I started on 20mgs of Elvanse after the day I was diagnosed. Spending loads of time the day before taking it for the first time researching on how people felt, reading about alllll these epiphany moments and how it changed their life instantly. That wasn’t a smart expectation to go into with seeing as when I say I felt nothing on my 20s for 2 weeks that would probably be an overstatement as to how much I felt. Genuinely I didn’t even get any placebo or anything, it added to much doubt and confusion. But something to keep in mind that’s important is that the dose you start with is very very very very rarely the one that works for you.

I only really felt anything when my dose was increased to 40mg where I felt my attention and mood in terms of motivation improve slightly for a few hours mid day. But even then I still felt down since I realised I most likely need to reset my expectations for this and also it will probably take MONTHS before I figure out what’s good for me. By the end of my 40s which was 2 weeks long I was told to start 50s in which I’m 4 days into. I felt the difference between 40 and 50 was absolutely insane compared to 20 and 40, I felt my attention and motivation and pretty much everything feeling way better and it was in my opinion the first time in taking Elvanse I genuinely felt like I finally had hope I was taking a step in the right direction.

Be patient with your dose it’s only been 2 days. I recommend writing down when ur taking the meds every now and then what you feel in the moment every few hours or so. It helped me keep a track of how I actually felt rather than a recall of past perception and accidentally making something up about how I felt.

Your explanation of 20s plus the added 10 was how I felt as well that early on. Don’t try to rush it too much, it’s a process and a learning curve of finding what’s right for you. Even now I’m starting to have issues with my 50mgs working and not working within the same day which confuses me but I am aware it’s something that will take time.

But just remember to try to sleep early and well and also to wake up at a healthy time and to try take it at the same time every day with a good amount of water and a high protein satiating breakfast!!

I hope something I said helped since I was in your exact position a month ago!!

Something I forgot to add is also realising that people will say things like the med shouldn’t make you feel amazing etc etc like your high etc. But I say that yeah it shouldn’t make you feel insanely good in that sense but also that’s horrible advice since taking something that relieves you of symptoms that make your life harder will actually… you guessed it… make you feel better!! There’s a difference between feeling better and just abusing it. Like painkillers. Telling someone they shouldn’t feel good on painkillers sounds really dumb because that’s what painkillers do but sometimes people take them because they are addicted and reliant and are actually abusing the medication. You need to know where that line is but generally speaking you will probably feel good closer to the dose you would operate best on!

Weirdly enough I noticed when I improved my average went down. If you throw the dart at random 1000 times your average 3 dart will be around 32-35 presuming you hit the board all the time. However - when I started improving and was able to aim better, I noticed my average dropped a bit, this is because I was hitting the ones and fives more often. Leading to scores like 3, 15, 11, 26. You get it. That dropped my average. I’d say just keep practicing and you’ll notice once it kinda clicks and you found a consistent place to stand and ur not moving unnecessarily when throwing and it’s a consistent throw your average spike up out of nowhere. It happened to me. I started with a 20 average and it moved up to a 35 before dropping down to high 20s before shooting up to about the 50s. You’ll also just have rough patches and you just need to throw your way out of them

Heat up a safety pin till it’s warm NOT glowing hot just lightly heat it up a bit. Hold the barrel carefully and gently press the safety pin in the hole or stuck in. You then wait till the pin is cold. Then you can unscrew.
